Why Blood Pressure and Cholesterol Matter More Than Almost Any Other Numbers

Cardiovascular disease (CVD) is the leading cause of premature death in the UK, responsible for around a quarter of all deaths (British Heart Foundation, 2025). Roughly one in seven men and one in twelve women in the UK die from coronary heart disease specifically. Stroke is the fourth leading single cause of death in the UK, with around 100,000 strokes each year.

The two most important modifiable risk factors for both heart disease and stroke are blood pressure and LDL cholesterol. Both can be measured and controlled. Both, when controlled to recommended levels, substantially reduce the risk of cardiac and cerebrovascular events. Yet around 31 percent of UK adults have hypertension (NICE, 2024) and an estimated half of those are either undiagnosed or undertreated.

A blood pressure and cholesterol check is the highest-yield single preventive intervention available to an adult under 75. If those numbers are at target, your cardiovascular risk is materially lower. If they are not, intervention is almost always effective.

What Should a Comprehensive Cardiovascular Risk Assessment Include?

A meaningful cardiovascular risk assessment in 2026 goes well beyond a single blood pressure reading and a total cholesterol test. The components below are the current evidence-based standard.

Blood pressure measured correctly. Seated, after five minutes of rest, both arms on the first visit, with a properly sized cuff. NICE recommends ambulatory or home blood pressure monitoring for diagnosis where clinic readings are above 140/90 (NICE NG136, updated 2024).

Full lipid profile. Total cholesterol, LDL cholesterol, HDL cholesterol, triglycerides and non-HDL cholesterol. Treatment thresholds in the UK are based on LDL or non-HDL, not total cholesterol.

HbA1c. Glycated haemoglobin gives a 3-month average of blood glucose and is the standard test for type 2 diabetes and pre-diabetes. Diabetes is itself a major cardiovascular risk factor.

High-sensitivity CRP and ESR. Markers of vascular inflammation, both independent predictors of cardiovascular events.

ECG. Resting 12-lead electrocardiogram to detect rhythm disturbances, prior silent infarction and conduction abnormalities.

QRISK3 score. The UK's standard 10-year cardiovascular risk calculator, combining age, sex, ethnicity, smoking, blood pressure, cholesterol, family history, BMI, chronic kidney disease, atrial fibrillation, rheumatoid arthritis, type 1 and type 2 diabetes, and several other factors. A score of 10 percent or above typically prompts discussion of statin therapy under current NICE guidance.

Echocardiogram and carotid Doppler. Where comprehensive imaging is included, these directly visualise heart structure and arterial plaque rather than relying on the bloods-and-blood-pressure proxy. This is the difference between probabilistic risk estimation and direct visualisation of disease.

What Are the Recommended Blood Pressure and Cholesterol Targets?

Under current NICE guidance (NG136, updated 2024) for adults under 80, the standard blood pressure target is below 140/90 in clinic or below 135/85 on ambulatory monitoring. For adults over 80, the target is below 150/90 in clinic.

For people with established cardiovascular disease, type 1 or type 2 diabetes, or chronic kidney disease, tighter targets may apply.

For LDL cholesterol, under NICE CG181 (updated 2024) primary prevention with a statin is offered to adults with a QRISK3 10-year cardiovascular risk of 10 percent or above. Secondary prevention (those with existing cardiovascular disease) targets LDL of below 2.0 mmol/L or a 40 percent reduction from baseline non-HDL, with high-intensity statin therapy.

These are population guidelines. Individual targets are agreed with your doctor after risk stratification.

What Happens If My Results Are Abnormal?

If blood pressure is raised, the standard pathway is ambulatory or home monitoring for confirmation, followed by lifestyle advice and consideration of antihypertensive medication if confirmed. If LDL is raised and your QRISK3 score is 10 percent or above, your doctor will discuss statin therapy. If HbA1c indicates pre-diabetes or diabetes, lifestyle and medication pathways follow.
