It is also the cancer where the screening conversation has changed most, because incidence in adults under 50 has been rising internationally for 3 decades while national programmes still begin at 50. In England the NHS bowel screening programme now starts at 50 and sends a home FIT kit every 2 years, and it is excellent, free and underused, so the first piece of advice on this page is to complete the kit the moment it arrives. The gap is everybody else. Under 50 with symptoms, under 50 with a family history, anyone with unexplained iron deficiency, anyone with a change in bowel habit lasting more than a few weeks, and the large group who have symptoms and have been managing them alone for a year. Those people need assessment now rather than an invitation in a decade. The tests that matter are specific. FIT detects blood invisible to the eye and is far more accurate than most people assume. Ferritin and iron studies catch the slow bleed that presents as tiredness. Faecal calprotectin separates inflammatory bowel disease from irritable bowel syndrome. Coeliac serology finds the diagnosis that hides behind both. And colonoscopy remains the only test that both diagnoses and treats. The symptoms that should never wait

A change in bowel habit lasting 3 weeks or more, particularly looser stools or increased frequency. Blood in the stool or on the paper, at any age, which people routinely attribute to piles for months. Unexplained weight loss. Abdominal pain that is new and persistent. A lump in the abdomen. And unexplained iron deficiency anaemia, which in men and postmenopausal women is treated by guidelines as gastrointestinal blood loss until proven otherwise.

Age does not exclude any of this. The reason under-50 bowel cancer is so often diagnosed late is that both patients and clinicians discount it on age, so symptoms get attributed to stress, diet, haemorrhoids or irritable bowel syndrome for a year or more. If you are 34 with rectal bleeding, you almost certainly do not have cancer, and you still need it explained rather than assumed.

Family history changes the threshold considerably. A first-degree relative diagnosed young, multiple affected relatives, or a family pattern including bowel, womb, ovarian or pancreatic cancer raises the possibility of Lynch syndrome and warrants genetic assessment and earlier, more frequent surveillance than any national programme provides.

What each test actually does

FIT, the faecal immunochemical test, measures human haemoglobin in stool and is the backbone of modern bowel assessment. It is quantitative, so the number matters rather than a simple positive or negative, and it is used both in screening and in deciding how urgently a symptomatic person needs a colonoscopy. It is not a cancer test and it does not exclude cancer on its own, which is why it belongs alongside assessment rather than instead of it.

Blood work adds the layer FIT cannot. Ferritin and iron studies find the slow chronic bleed that has already caused anaemia. Full blood count, inflammatory markers, liver and kidney function, and coeliac serology, because adult coeliac disease presents as fatigue and iron deficiency far more often than as dramatic gut symptoms. Faecal calprotectin measures gut inflammation and is the test that separates inflammatory bowel disease from irritable bowel syndrome, which spares a great many young people an unnecessary colonoscopy and sends the right ones straight to it.

Colonoscopy is the definitive investigation, because it visualises the whole colon, takes biopsies and removes polyps in the same procedure. That last point is the one people miss. A colonoscopy is not only diagnostic, it is preventive, since every adenoma removed is a cancer that will now never happen.

How the assessment runs here

A GMC-registered doctor takes the history that decides everything, symptoms, duration, family history, medication, iron status and previous investigations, and examines you. That conversation determines whether you need reassurance and monitoring, a stool and blood workup, or a colonoscopy this month.

Testing is then organised on your behalf. FIT and faecal calprotectin through accredited laboratories, bloods including ferritin and iron studies, coeliac serology and inflammatory markers in the same visit, and where imaging or endoscopy is indicated it is booked at the London centre best equipped for it, with consultant gastroenterologists and consultant radiologists reporting.
