Finding a lump is one of the most frightening things that sends people to a doctor, and the reassuring fact comes first. The great majority of lumps are benign. That said, a lump is the one symptom where waiting to see what happens is the wrong instinct, because the few that matter are far more treatable found early, and because the relief of knowing is worth having in days rather than months.

Which lumps need checking this week
Any breast lump in a woman or a man, and any breast change including skin dimpling or puckering, a nipple that has newly turned in, bloodstained nipple discharge, or a persistent rash on the nipple. Most will be benign, and all deserve assessment rather than watching.
Any testicular lump, swelling, heaviness or change in shape, at any age. Testicular cancer is the commonest cancer in men aged 15 to 45 and is among the most curable when caught early, so a new testicular lump is examined and scanned promptly rather than reassured over the phone.
A neck lump persisting beyond 3 weeks, particularly in anyone over 45, a smoker, or with hoarseness, difficulty swallowing, ear pain on one side or unexplained weight loss, since these features place it on the head and neck cancer pathway in NICE guideline NG12.
Any lump that is hard, fixed rather than mobile, irregular, growing, painless, larger than about 5cm, or deep beneath the muscle rather than just under the skin. And any lump accompanied by drenching night sweats, unexplained weight loss, persistent fever or generalised itching, which raises lymphoma.
A lump in the groin that appears when you cough or strain and disappears when you lie down is usually a hernia, and one that becomes painful, tense and will not go back needs same-day assessment because it may be strangulated.
What most lumps turn out to be
In the breast, fibroadenomas are firm, smooth, mobile and common in younger women. Cysts are fluid-filled, often appear or change with the menstrual cycle, and are commonest in the 40s. Fibrocystic change produces lumpy, tender breasts that vary through the month. Fat necrosis follows injury. All are benign, and ultrasound distinguishes them reliably from the lumps that need biopsy.
Under the skin, lipomas are soft, rubbery, slow-growing fatty lumps that move freely. Epidermoid cysts are firm, round and often have a small central punctum. Both are harmless and can be removed if bothersome, with histology sent routinely.
In the neck, reactive lymph nodes swell during and after an infection, are often tender, and settle within 2 to 3 weeks. Thyroid nodules are found in around 50% of adults on ultrasound and over 95% are benign.
In the testis, epididymal cysts, varicoceles and hydroceles are common and benign, and ultrasound separates them clearly from a solid testicular mass.
The point of assessment is not that most lumps are dangerous. It is that ultrasound tells you which category yours falls into within days, which ends the worry for the majority and starts treatment early for the few.

How a lump is investigated
Examination first, because size, texture, mobility, tenderness and the nodes around it tell an experienced clinician a great deal and determine which imaging is needed.
Ultrasound is the first-line investigation for most lumps. It uses no radiation, characterises solid versus cystic, measures blood flow and assesses margins, and it is arranged at the London centre best equipped for that region, performed by an experienced sonographer and reported by a consultant radiologist. Breast assessment in women over 40 usually adds mammography.
Where imaging shows features needing tissue diagnosis, image-guided biopsy is arranged, meaning a needle guided by ultrasound into the lump to obtain cells or a core of tissue for the pathologist. This is quick, usually done under local anaesthetic, and is what separates benign from malignant with certainty.
Blood tests at an accredited partner laboratory where the picture warrants them, including full blood count and inflammatory markers for suspected lymphoma or infection, thyroid function for neck lumps, and tumour markers for testicular masses.
Then referral to a named consultant breast, head and neck, urology or general surgeon where the findings require it, with the appointment booked rather than suggested, and a cancer pathway escalated the same day where one is indicated.
