Full Health Check in Your 30s. What to Test, What to Watch For and Why This Decade Matters

Your 30s are the decade in which most long-term health risks begin to build, usually without symptoms, and the NHS Health Check does not start until 40. Blood pressure, cholesterol and blood sugar can drift upwards for years before anyone measures them. Lipoprotein(a), a genetically set risk factor for heart attack and stroke that is raised in around 1 in 5 people, can be measured once in adult life and never needs repeating. Early-onset bowel cancer is rising, with rates in people aged 25 to 49 in England increasing by 3.6% a year in the decade studied, which makes knowing the warning signs more important than ever. For women, fertility declines with age and more steeply after 35, and cervical screening continues every 5 years with HPV testing. For men, testicular cancer is most common between 15 and 49, and self-examination takes a minute. Why have a health check in your 30s

Because the numbers that predict heart disease, diabetes and liver disease in your 50s are already visible in your 30s. They change slowly, and the earlier they are found the smaller the change needed to correct them.

Because family history starts to mean something. If a parent had a heart attack, diabetes or cancer young, your own risk is higher, and the right tests can be chosen around it.

And because a baseline is valuable in itself. A cholesterol, blood sugar or kidney result taken at 34 makes every later result easier to interpret, since a doctor can see the direction of travel rather than a single number.

What should a full health check in your 30s include

A detailed history and examination. Family history, lifestyle, sleep, alcohol, exercise and stress, then blood pressure, pulse, weight and waist measurement.

Heart and circulation. A lipid profile with apolipoprotein B, and lipoprotein(a) measured once. HEART UK recommends testing lipoprotein(a) at least once in adult life, and a raised level changes how other risk factors are managed.

Blood sugar. HbA1c, which reflects average glucose over around 3 months and identifies prediabetes while it is still reversible.

Liver and kidneys. Liver function tests and kidney function, with a urine test for protein where blood pressure or blood sugar are raised.

Blood count, iron and vitamins. A full blood count, ferritin, vitamin B12, folate and vitamin D. Iron deficiency is common in women with heavy periods and is a frequent cause of tiredness.

Thyroid and hormones where they are indicated. Thyroid function when there are symptoms such as tiredness, weight change or irregular periods, and hormone tests when the history points to them.

What should women in their 30s know about fertility

Fertility declines with age, gradually through the early 30s and more steeply after 35. NICE advises that around 8 in 10 couples where the woman is under 40 will conceive within 1 year of regular unprotected sex.

AMH, the blood test often sold as a fertility test, reflects the number of eggs remaining and is useful in planning IVF. A study in JAMA in 2017 found that it did not predict the chance of conceiving naturally in women trying to conceive. A low result is not a reason to panic, and a normal result is not a guarantee.

What helps most is a conversation with a doctor about your plans, your cycle and your history, with tests chosen to answer your question. See a doctor after 12 months of trying, or after 6 months if you are 36 or over or have irregular periods.

Which cancer warning signs matter in your 30s

Bowel cancer is becoming more common in younger adults. Research published in The Lancet Oncology in 2024 found early-onset bowel cancer rising in 27 of 50 countries studied, including England. See a doctor promptly for blood in your stools, a lasting change in bowel habit, unexplained weight loss, persistent abdominal pain or unexplained tiredness from iron deficiency.

Testicular cancer is most common between 15 and 49. Check once a month for a lump, swelling or heaviness, and see a doctor about any change.

Breast changes should be checked at any age. A new lump, a change in shape, skin dimpling or nipple changes all need examining.

Skin cancer is linked to sun exposure and sunburn. Any mole that changes in size, shape or colour, bleeds or itches should be examined with dermoscopy.

Cervical screening continues from 25 to 64. In England, people aged 25 to 49 whose HPV test is negative are now invited every 5 years.

How often should you have a health check in your 30s

For most healthy people in their 30s with no family history, a full baseline once, then every 3 to 5 years, is reasonable. Every 1 to 2 years makes more sense if your blood pressure, cholesterol, blood sugar or weight are borderline, if you have a strong family history, or if you are planning a pregnancy.

Sooner if something changes. New symptoms, a new diagnosis in a parent or sibling, or a significant change in weight are all good reasons to recheck.
