Because the arithmetic changes. Cardiovascular risk climbs steeply through the fifties in both sexes, and sharply in women after menopause as oestrogen's protection recedes. For a free service delivered at population scale it is well designed and it catches a great deal. What it does not include is imaging of any kind, a full thyroid panel, ferritin, vitamin D, B12, hormonal assessment, or the time for a detailed family history. It also runs on a five-year cycle, which is a long gap in the decade when things actually move. The honest position is that the NHS Health Check is the floor rather than the ceiling. Attend it, attend your screening programmes, and if you want the layer above, that is what a doctor-performed assessment provides.

The most extensive full-body imaging programmes in London exceed £32,000, Hooke in Mayfair sells membership reported up to £25,000, and Echelon Health runs from around £2,000 to beyond £15,000 at Platinum. Bupa's Mature Health assessment, aimed at exactly this age group, is priced at around £1,009, with its fullest assessments reaching around £1,199, and Nuffield Health's 360 plus around £1,129. Nurse-led venue screening runs from roughly £169 to £319, with postal kits beneath it, defined by the absence of a doctor.

Three reasons that survive scrutiny. The imaging comes first. A doctor-performed ultrasound examines the abdominal aorta fifteen years before the NHS looks at it, and looks at it in women, whom the NHS programme does not screen at all. Then the interpretation.
