Longevity testing in London- What it costs, and which tests matter

A comprehensive biomarker panel has become one of the most talked-about tools in preventive medicine, and also one of the most inconsistently priced. In London, a serious blood-based health assessment can run from a few hundred pounds to £1,000 to £2,000 for a genuinely comprehensive panel, while imaging-heavy executive health days can reach £14,000 to £15,000, and premium longevity memberships are now marketed at more than £50,000 a year, both in London and internationally at figures approaching $100,000.

The price tells you almost nothing about the medicine. What matters is which biomarkers are actually measured, whether a doctor interprets them against your personal history, and whether the result is a plan you can act on rather than a spreadsheet of numbers with red and green flags.

What a longevity assessment is actually trying to do

The useful definition is simpler than the marketing. A good longevity assessment should identify modifiable risks that may affect how well you live over the next ten, twenty and thirty years, establish a baseline while you are well, and give you something measurable to improve.

That means cardiovascular risk before cardiovascular disease. Metabolic dysfunction before diabetes develops. Nutritional deficiency before it becomes symptomatic. It means identifying things that can reasonably be measured, interpreted and acted on.

The objective is not to prove that you are biologically younger than your passport says. No single number from a blood panel, biological age score or algorithm should be treated as a verdict on how long you will live.

The biomarkers worth knowing before you pay for the exotic ones

A sophisticated-looking report can contain hundreds of measurements while still missing some of the markers that matter most.

For cardiovascular risk, a standard cholesterol panel is useful but incomplete. ApoB reflects the number of atherogenic particles circulating in the blood. Two people can have an identical LDL cholesterol level while carrying very different numbers of these particles.

Lipoprotein(a), or Lp(a), identifies an important and largely genetically determined cardiovascular risk factor that may not be obvious from LDL cholesterol alone. Because Lp(a) tends to remain relatively stable throughout life, it generally does not need to be repeatedly measured in the same way as cholesterol.

Homocysteine and hs-CRP can add further information about cardiovascular and inflammatory risk when interpreted in the context of the rest of the clinical picture.

Metabolic health deserves the same attention. Fasting insulin may become elevated while fasting glucose remains normal because the body initially compensates for insulin resistance by producing more insulin. HbA1c provides a longer-term picture of glucose exposure. Read together rather than individually, these can tell you more about the direction metabolic health is moving.

Then come the measurements that can help explain how you actually feel. Full blood count and iron studies. Thyroid function. Vitamin B12 and folate. Vitamin D. Magnesium, zinc and selenium. Sex-specific hormonal markers where clinically relevant. Kidney and liver function.

More comprehensive panels can go further, using markers such as cystatin C for kidney function, liver fibrosis markers and measures such as the omega-3 index.

A good longevity panel measures enough to identify useful patterns without pretending that every measurable molecule has been proven to predict lifespan.

Composite biological age scores can also be useful for following your own trend over time and visualising whether measured biology appears better or worse than expected for age. But they are not a diagnosis, do not guarantee lifespan, and should not replace interpretation of the underlying results by a physician.

What longevity testing costs at The Wellness

At the other end of the London private-health market, executive health assessments combining extensive biomarker testing, imaging and specialist consultations can run into several thousand pounds, with some imaging-heavy assessments reaching around £14,000 to £15,000. Premium concierge longevity memberships can be marketed at more than £50,000 a year.

The important distinction is that you do not need to spend £10,000 or £15,000 simply to gain useful longevity data, and you do not need to buy an annual five-figure membership simply because a clinic uses the word longevity.

The 5 questions to ask any longevity clinic before paying

Ask what decisions will actually change because of the tests being performed. If nobody can explain what will be done differently as a result, you may be buying information rather than medicine.

Ask who interprets everything together. A cardiology result, hormone panel and nutrient marker should not necessarily be treated as separate spreadsheets when one physician can interpret the complete clinical picture.

Ask whether ApoB and Lp(a) are measured. A longevity programme with an enormous biomarker list but no meaningful cardiovascular risk assessment may have its priorities backwards.

Ask which recommendations are established medical care and which are optimisation or experimental interventions. The distinction should be made clearly.

And ask what happens after the results arrive. The most detailed panel in the world has limited value if the final interaction is simply an automated portal showing which numbers are red, amber or green.

How often should longevity biomarkers be repeated?

There is little value in repeatedly measuring every marker simply because it can be measured.

Markers that respond to lifestyle change, including fasting insulin, HbA1c and inflammatory markers, may reasonably be reassessed around every six to twelve months when someone is actively working to improve them.

Lp(a), because it is predominantly genetically determined and usually relatively stable, generally only needs to be measured once unless there is a clinical reason to repeat it.

Thyroid and hormonal testing may be reviewed annually or sooner where symptoms, treatment or circumstances change.

Testing too rarely can miss an emerging trend. Testing too frequently can turn normal biological variation into apparent abnormalities. The appropriate interval should therefore be determined by the clinical context rather than by habit.
