Where in the World Is Meningitis a Risk for Travellers?
Meningococcal disease exists worldwide, but certain regions carry substantially higher risk than the UK. The African meningitis belt stretches across sub-Saharan Africa from Senegal and The Gambia in the west through Mali, Burkina Faso, Niger, Nigeria, Chad, Central African Republic, South Sudan, and Ethiopia in the east. This region experiences regular epidemics of meningococcal disease, particularly during the dry season (DecemberβJune). Outbreaks can involve tens of thousands of cases. Group A has historically been the dominant strain in this region, though groups W and C are also significant. MenACWY vaccination is strongly recommended for any traveller to this area. Saudi Arabia requires MenACWY vaccination for all Hajj and Umrah pilgrims. The mass gathering of millions of people from around the world creates ideal conditions for meningococcal transmission, and outbreaks have occurred historically at the Hajj. Your vaccination certificate must show that the vaccine was given within the previous 5 years and at least 10 days before arrival in the Kingdom. Parts of the Middle East and South Asia carry elevated meningococcal risk, particularly during religious festivals and mass gatherings. University campuses worldwide β if you or your child is studying abroad (particularly in the US, where MenB outbreaks on college campuses have been documented), vaccination against both MenACWY and MenB is strongly recommended. Which Meningitis Vaccine Do Travellers Need?
MenACWY is the primary travel meningitis vaccine. It covers the four strains (A, C, W, Y) most commonly associated with meningococcal epidemics outside the UK. It is the vaccine required by Saudi Arabia and recommended by travel health authorities for the African meningitis belt and other high-risk destinations. A single dose provides protection for approximately 5 years. It can be given to adults and children from 6 weeks of age. MenB (Bexsero) is not routinely required for travel but should be considered in certain circumstances: travel to an area experiencing a MenB outbreak, extended stays involving close-contact living arrangements (dormitories, volunteer camps, shared housing), healthcare work in areas where MenB is prevalent, and for travellers who simply wish to be comprehensively protected. MenB requires two doses (at least 4 weeks apart on the accelerated schedule). For most travellers, MenACWY alone is sufficient. For university students abroad, backpackers in communal accommodation, or those heading to MenB-prevalent regions, adding Bexsero provides broader coverage. Hajj and Umrah: Meningitis Vaccination Requirements
Saudi Arabia has mandatory meningococcal vaccination requirements for all pilgrims. The current requirements are that all travellers aged 2 years and over entering Saudi Arabia for Hajj or Umrah must provide proof of MenACWY vaccination (quadrivalent vaccine covering groups A, C, W, and Y). The vaccine must have been given at least 10 days before arrival and within the previous 5 years. An International Certificate of Vaccination or Prophylaxis (ICVP) is required as proof.
