The largest randomised trial ever run on a cancer blood test reported in 2026, and almost everything written about it since has been either too optimistic or too dismissive. Here is what NHS-Galleri actually showed. 142,924 people aged 50 to 79 in England were randomised, with blood taken at up to 3 annual visits. The trial missed its primary endpoint, which was a reduction in combined stage 3 and stage 4 cancers. It did reduce the number of cancers found at stage 4, the stage at which cure is rarely possible, with the effect growing across successive screening rounds. It produced a four-fold higher cancer detection rate, increased detection of deadly cancers at stages 1 and 2, and reduced the number of cancers presenting through symptoms or emergencies, which are the presentations that carry the worst outcomes. Professor Charles Swanton described the technology as showing real clinical promise while being clear that the predefined endpoint was not met. The practical conclusion is that a multi-cancer blood test is a genuine addition to a screening programme and a poor substitute for one. How the test works and what the numbers mean
Tumours shed fragments of DNA into the blood, and the methylation patterns on those fragments differ from healthy cells. A multi-cancer early detection test looks for that shared cancer signal and, where one is found, predicts the likely tissue of origin so that investigation can start in the right place. More than 50 cancer types can produce a signal, and around 47 of them have no screening programme in the UK at all, which is where the appeal sits.
The performance numbers matter more than the marketing. Specificity is high, with a false positive rate under 1 percent, which is what makes the test usable at scale. Sensitivity is the harder number, because it rises steeply with stage, so the test is considerably better at finding advanced cancers than early ones, and that is the honest limitation behind the trial result.
The most useful way to read NHS-Galleri is that annual testing shifted when cancers were found rather than eliminating late diagnosis. Fewer stage 4 presentations, fewer emergency presentations, four times as many cancers detected overall. That is worth having. It is not a reason to stop doing anything else.
Who it suits, and who should not buy it
The strongest case is a person over 50, already using every national programme they are invited to, with a family history that worries them, who wants coverage of the cancers no programme screens for, and who understands that a negative result reduces rather than removes risk. Used that way it adds something real.
The weakest case is anyone using it as a substitute. A negative multi-cancer test does not mean you can skip cervical screening, breast screening, bowel screening or a prostate conversation, and it certainly does not mean a symptom can be ignored. A symptom needs a diagnosis, not a screening test, and that is the single most important sentence on this page.
Two groups should think carefully before testing. People who would be destabilised by an inconclusive or false positive result, since a signal detected without a located tumour leads to a full diagnostic workup that occasionally finds nothing. And anyone under 50 with no risk factors, where the pre-test probability is low enough that the arithmetic of false positives works against you.
