Performance Anxiety and Stage Fright -What Actually Helps and What Doctors Prescribe
A racing heart, shaking hands, a dry mouth and a mind that goes blank at the worst possible moment are the physical signature of performance anxiety, and they affect people who are otherwise entirely confident, competent professionals, not just naturally nervous ones. Situational performance anxiety, the kind triggered specifically by a wedding speech, a board presentation, an exam, a courtroom appearance or walking on stage, is one of the most common reasons people seek a short-term prescription privately, and it is also one of the most poorly understood, since the medication most people assume they need is not the one doctors generally reach for. Beta blockers, used off-label for situational performance anxiety, control the physical symptoms of adrenaline, the shaking, the racing heart and the tremor in the voice, without causing sedation or affecting mental clarity, which is precisely why they are the standard first-line option for this specific kind of anxiety rather than a benzodiazepine. At The Wellness you are properly assessed, including for the small number of conditions that make beta blockers unsuitable, and a plan is made that is trialled in advance of the day itself rather than for the first time when it matters most. Consultations from £150 by video and £220 in person. Fees appear further down this page.

What is performance anxiety and why does it feel so physical
Performance anxiety is driven by an adrenaline surge in response to a specific, identifiable trigger, a stage, a presentation, an exam or a similar high-stakes situation, rather than the more diffuse and constant worry seen in generalised anxiety disorder. That adrenaline response produces the classic physical symptoms, a pounding or racing heart, trembling hands, a shaking voice, sweating, a dry mouth and sometimes nausea or a mind that briefly goes blank, and it is these physical symptoms, more than the underlying worry itself, that people find most disruptive and most visible to an audience.
It is worth distinguishing this from generalised anxiety disorder or a broader anxiety condition, which produces worry and physical symptoms across many situations rather than 1 specific trigger, and which is generally managed quite differently, often with an entirely different class of medication and with psychological therapy as a central part of treatment rather than a one-off situational plan.
Why beta blockers rather than benzodiazepines
Beta blockers, such as propranolol, work by blocking the effect of adrenaline on the heart and blood vessels, which is exactly the physical loop that performance anxiety runs on. They do not sedate, do not affect concentration or memory, do not impair the ability to think clearly or respond to questions, and they carry no dependency risk, which makes them well suited to an occasional, planned use ahead of a specific event rather than an ongoing daily medication.
Benzodiazepines are generally avoided for this specific purpose. They are sedating, which can blunt exactly the sharpness a presentation or an exam requires, they carry a real risk of dependency with repeated use, and their effects can be unpredictable in terms of timing and intensity in a way that is a poor fit for a single, high-stakes performance where reliability matters more than almost anything else. Where a doctor is asked specifically for something to take before a big presentation, a beta blocker is very often the answer they will discuss first, precisely because of this difference in profile.
Who beta blockers are and are not suitable for
Beta blockers are not suitable for everyone, and this is exactly what a proper assessment is for rather than a self-directed decision. Asthma and some other respiratory conditions are a significant consideration, since beta blockers can worsen airway constriction in susceptible people. Certain heart conditions, particularly slow heart rhythms or specific conduction problems, and very low blood pressure also need to be identified before prescribing. A history of diabetes is relevant too, since beta blockers can mask some of the warning signs of low blood sugar.
None of this makes beta blockers rare or exotic, they are one of the most widely prescribed classes of medication in the country, but it does make a proper history and a check of blood pressure and pulse a necessary first step rather than something to skip because the medication feels routine.

Why the plan needs trialling before the day
The single biggest mistake people make with situational anxiety medication is taking it for the first time on the day it actually matters. Individual response varies, and while side effects are generally mild and uncommon, the sensible and standard approach is a trial run ahead of a lower-stakes situation, so you know how the medication affects you, what timing works best for you personally, and that it does what you need it to do, well before you are relying on it for the presentation, the speech or the exam itself.
This is discussed as part of the consultation, along with the timing that suits your particular event, and a written plan is provided so there is no uncertainty on the day about what to take or when.
What else actually helps
Medication addresses the physical symptoms, but it works best alongside, rather than instead of, the techniques that address the anxiety itself. Preparation and rehearsal reduce anxiety more reliably than almost anything else, since uncertainty about content is a major driver of the physical response. Controlled breathing techniques, practised in advance rather than attempted for the first time under pressure, measurably reduce the physical symptoms of an adrenaline surge.
For anxiety that goes well beyond an occasional presentation, recurs across many situations, or is accompanied by broader worry, low mood or avoidance of situations that matter to you, cognitive behavioural therapy has strong evidence specifically for performance and social anxiety, and is discussed as a more durable option where the pattern extends beyond a single event. Where anxiety is more generalised or persistent, a different and more thorough assessment, rather than a one-off situational plan, is the right next step, and this is identified in the consultation rather than treated the same as an occasional nervous presentation.
What does assessment cost in London
Private GP appointments for a short-term prescription in central London commonly run £120 to £250, and where the assessment is rushed or done entirely by questionnaire without checking blood pressure, pulse, or a personal history of asthma or heart problems, the medication is being prescribed without the safety check that actually justifies needing a doctor for it in the first place. Ongoing therapy for broader anxiety typically runs £100 to £150 per session privately in London.
Same-day video consultation for an occasional performance anxiety plan, from £150. In-person consultation with blood pressure and pulse check in Marylebone, from £220. Extended 45-minute consultation where anxiety extends beyond a single event or a fuller history is needed, from £395. Home, hotel or office visit from £495 daytime and £695 evenings and weekends, for anyone who prefers to be seen ahead of travel or a specific commitment. Prescribing, a written same-day plan, and referral for cognitive behavioural therapy or a fuller anxiety assessment arranged where indicated. Same-day appointments 7 days a week in Marylebone, 3 minutes from Baker Street, so an assessment ahead of a presentation can be arranged even at short notice.

Frequently asked questions
What medication do doctors actually prescribe for stage fright
Beta blockers, most commonly propranolol, used off-label for situational performance anxiety, are the standard first-line option because they control the physical symptoms of adrenaline without sedating you or affecting mental clarity, unlike a benzodiazepine.
Why not just take a benzodiazepine
Benzodiazepines are sedating, which can dull the sharpness a presentation or exam requires, carry a real risk of dependency with repeated use, and their timing and intensity can be less predictable, which is a poor combination for a single high-stakes event where reliability matters most.
Is it safe to take a beta blocker before a presentation
For most people, yes, once a doctor has confirmed you do not have asthma, a significant heart rhythm problem, very low blood pressure or another reason it would not suit you, which is exactly what a proper assessment checks before anything is prescribed.
What if my anxiety is not just about 1 event
If anxiety extends across many situations, or comes with broader worry, low mood or avoidance, that points toward a fuller assessment and often cognitive behavioural therapy rather than a one-off situational plan, and this is identified in the consultation.
Can I get this arranged at short notice before a presentation
Yes, same-day appointments are available 7 days a week, and a home, hotel or office visit can be arranged where travel to the clinic is not practical ahead of a specific commitment.
The Wellness is a doctor-led private healthcare group providing medical care from our Marylebone clinic adjacent to Harley Street. All doctors are GMC-registered.
