Menopause and Perimenopause
Menopause remains one of the most under-discussed and under-treated health transitions women experience. The average age of menopause in the UK is 51, but perimenopause — the years of hormonal fluctuation leading up to it — can begin in your early 40s or even late 30s. Symptoms can significantly impact your quality of life, your work, and your relationships.
Hot flushes, night sweats, mood changes, anxiety, brain fog, joint pain, disrupted sleep, reduced libido, vaginal dryness, and weight gain are among the most common symptoms. Many women do not realise these symptoms are connected to hormonal changes, particularly when they begin in their 40s — years before periods actually stop.
What we offer for menopause and perimenopause. Our doctors will conduct a thorough assessment of your symptoms, medical history, and risk factors. We can arrange hormonal blood tests (FSH, LH, oestradiol, progesterone, thyroid function, and others as indicated) to help confirm the picture, although menopause and perimenopause are primarily diagnosed based on symptoms and age rather than blood tests alone.
If HRT is appropriate for you, we prescribe it — discussing the different types (oestrogen-only, combined oestrogen and progesterone, body-identical hormones, patches, gels, tablets, and vaginal treatments), the benefits, the risks, and what to expect. We follow current NICE guidelines which confirm that for most women the benefits of HRT outweigh the risks when started under 60 or within 10 years of menopause.
If HRT is not suitable or not desired, we discuss alternative approaches including non-hormonal medications, lifestyle modifications, and when appropriate, referral to a menopause specialist.
We provide ongoing monitoring — typically a follow-up at 3 months after starting HRT, then every 6-12 months — to review symptoms, adjust dosing, and ensure the treatment continues to work for you.
Contraception
Getting contraception should not require a two-week wait. At The Online GP, we provide same-day consultations for all hormonal contraception methods.
Combined oral contraceptive pill. We prescribe after reviewing your medical history, blood pressure, BMI, and any risk factors for blood clots, stroke, or breast cancer. For patients already taking a pill that suits them, we can issue continuity prescriptions.
Progesterone-only pill (mini-pill). Suitable for women who cannot take combined contraception due to migraine with aura, high blood pressure, BMI over 35, or other contraindications. Same-day prescription available.
Emergency contraception. EllaOne (ulipristal acetate, effective up to 120 hours after unprotected sex) and Levonelle (levonorgestrel, effective up to 72 hours) can both be prescribed during a same-day consultation.
Contraceptive advice and switching. If your current method is causing side effects or is not working for you, our doctors will discuss alternatives and help you switch safely.
For long-acting methods such as the coil (IUD/IUS), implant, or injection, we provide advice and referrals to experienced fitting clinics.

Polycystic Ovary Syndrome (PCOS)
PCOS affects approximately 1 in 10 women in the UK, yet it is frequently underdiagnosed and poorly managed. Symptoms include irregular or absent periods, acne, excess hair growth (hirsutism), weight gain (particularly around the abdomen), hair thinning, difficulty conceiving, and mood changes.
Diagnosis requires at least two of three criteria: irregular ovulation (irregular periods), clinical or biochemical signs of excess androgens (acne, hirsutism, or elevated testosterone on blood tests), and polycystic ovaries on ultrasound.
Our doctors can arrange the blood tests needed to investigate PCOS — including testosterone, SHBG, LH, FSH, DHEAS, insulin, glucose, HbA1c, thyroid function, and prolactin — and refer for ultrasound if indicated. We then create a management plan that may include lifestyle modifications, the combined pill (which helps regulate periods and reduce androgen levels), metformin (for insulin resistance), or specialist referral for fertility support.
Hormonal Health and Blood Testing
Many symptoms that women attribute to stress, ageing, or "just getting on with it" are actually hormonal. Fatigue, mood changes, weight changes, hair loss, skin problems, low libido, and irregular periods all have hormonal components that can be identified through blood testing.
Our women's hormonal panel includes oestradiol, progesterone, testosterone, SHBG, FSH, LH, prolactin, DHEAS, thyroid function (TSH, free T4, free T3, thyroid antibodies), iron studies, vitamin D, B12, and folate. Pricing starts from £99 for a targeted panel and £199 for a comprehensive hormonal screen.

Cervical Screening (Smear Tests)
NHS cervical screening is offered every 3 years for women aged 25-49 and every 5 years for women aged 50-64. But many women miss their screening — sometimes because of anxiety, sometimes because they cannot get an appointment, and sometimes because they are not registered with an NHS GP.
Private cervical screening at our Marylebone clinic is available for women who have missed their NHS screening, prefer the option of requesting a female doctor, want results faster than the NHS turnaround, or are not registered with an NHS GP (international patients, recent arrivals to the UK).
Other Women's Health Services
Urinary tract infections (UTIs). Same-day assessment and antibiotic prescription when appropriate, without waiting for an NHS appointment. For recurrent UTIs, we investigate underlying causes and discuss preventive strategies.
Vaginal and vulval concerns. Thrush, bacterial vaginosis, vulval irritation, and changes in discharge can all be assessed and treated. Video consultation is often sufficient for straightforward cases; in-person examination is available when needed.
Pre-conception health. Planning a pregnancy? We provide pre-conception health checks including blood tests for rubella immunity, thyroid function, iron levels, and folic acid advice. We review any current medications for safety in pregnancy and optimise your health before conception.
Fertility concerns. While we are not a fertility clinic, we provide initial investigations for women who are struggling to conceive — including hormonal blood tests, ovulation assessment, and referral to fertility specialists when indicated.
Breast concerns. Breast lumps, pain, and changes should always be assessed. We provide clinical examination and urgent referral when warranted.
