Four different problems, one word
Shoulder pain is not a diagnosis, and the shoulder is the joint where guessing goes wrong most often. Four conditions account for the overwhelming majority of cases, they respond to completely different treatment, and they are told apart in minutes with a scan and an examination.
Rotator cuff tendinopathy and subacromial pain is the most common. It hurts on reaching overhead, on lying on that side at night, and on the arc between roughly sixty and one hundred and twenty degrees of lifting. Movement is painful but achievable.
A rotator cuff tear, partial or full thickness, presents similarly but with weakness rather than just pain, particularly on holding the arm out or rotating against resistance. This distinction matters enormously, because a full thickness tear in a younger or active patient is a surgical conversation, not an injection one.
Frozen shoulder, or adhesive capsulitis, is the one people misidentify most. The hallmark is loss of passive external rotation, meaning someone else cannot rotate your arm outward either. If the shoulder will not move even when you are relaxed and someone else is moving it, the problem is the capsule, not the tendons.
Acromioclavicular joint pain sits pinpoint on the top of the shoulder, hurts on reaching across the body, and is often ignored because attention goes to the cuff underneath it.
Add calcific tendinopathy, where a chalky deposit forms in the tendon and produces some of the most severe shoulder pain in medicine, and you have the map.
Why two failed injections usually means the wrong target
Patients arrive regularly having had two or three injections that did nothing. The natural conclusion is that injections do not work for them. The more likely explanation is that the medication was delivered to the right general area and the wrong specific space.
The subacromial bursa, the glenohumeral joint and the acromioclavicular joint are all treatable and all separate. Unguided shoulder injections have among the lowest published accuracy rates of any site in the body, because the target is small, deep and surrounded by structures that look identical from the outside. Under ultrasound the bursa is visible as a distinct layer, the needle is watched into it, and the fluid is seen distributing where it should.
The second reason injections fail is that the diagnosis was wrong from the start. Steroid into the subacromial space does very little for a frozen shoulder, because the pathology is in the capsule beneath it, and it does nothing at all for a full thickness cuff tear except delay the decision.

What shoulder ultrasound actually shows
Ultrasound is the appropriate first-line imaging for most shoulder pain, and in several respects it outperforms static imaging because it looks at the shoulder moving.
It shows the rotator cuff tendons in detail, distinguishing tendinopathy from partial thickness and full thickness tears. It shows calcific deposits and their size and consistency, which determines whether barbotage is worth attempting. It shows the subacromial bursa and whether it is thickened or distended. It shows the acromioclavicular joint and whether it is arthritic or inflamed. It shows the long head of biceps tendon, a frequent and frequently missed pain generator. And with the arm moving under the probe, it shows impingement happening in real time.
What it does not show well is the labrum and the deep joint surfaces, so a suspected labral tear, instability or significant arthritis is a case for MRI or orthopaedic referral rather than repeated local treatment. Saying so plainly is part of the assessment.

Treating frozen shoulder properly
Frozen shoulder follows a predictable pattern that dictates treatment. The freezing phase is dominated by pain, often severe at night, with movement progressively closing down over roughly two to nine months.
