When Do You Need a Fit-to-Fly Certificate?
While COVID-related travel health certificates are largely a thing of the past, airlines still require medical clearance in several common situations:
Pregnancy. Most airlines require a fit-to-fly letter from 28 weeks of pregnancy for single pregnancies and from 24 weeks for multiple pregnancies. Some airlines won't allow travel after 36 weeks (32 weeks for multiples) regardless of medical clearance. Your certificate should confirm your due date, that the pregnancy is uncomplicated, and that the doctor considers you fit to fly.
Recent surgery or medical procedure. Airlines typically require medical clearance if you've had surgery within the past 7-14 days, depending on the procedure. Abdominal surgery, orthopaedic procedures, cardiac interventions, and eye surgery are common triggers.
Chronic medical conditions. If you have a heart condition, respiratory disease (particularly if you require supplemental oxygen), a blood clotting disorder, recent stroke, or severe anaemia, your airline may request medical documentation.
Infectious disease recovery. If you've recently had a notifiable infectious disease, airlines may require confirmation that you're no longer contagious.
Travelling with medical equipment. If you need to carry a CPAP machine, portable oxygen concentrator, injectable medications, or other medical devices, a doctor's letter confirming their medical necessity helps avoid issues at security and with airline staff.
Plaster casts and mobility aids. Airlines have specific policies about flying with plaster casts (particularly within 24-48 hours of application) and may require medical clearance.
