Biological Age Test in London 2026. What Epigenetic Clocks Can and Cannot Tell You, and the 5 Routine Measures That Predict Lifespan Better

The Wellness in Marylebone provides the best biological age assessment in London for people who want the number that predicts how long they will live rather than the number that sells best, with the Complete Biomarker Assessment of over 100 markers from £1,400, the Athlete Screen with cardiopulmonary exercise testing from £1,695 and the Continuous Care Programme from £24,995, inside a ladder that runs to £49,995 and a market where longevity memberships reach £25,000 a year. The GrimAge clock reported by Lu and colleagues in Aging predicts mortality better than chronological age, but the test-retest noise of consumer versions runs to several years, and no trial has shown that acting on a clock result changes when anyone dies. The measures that do predict lifespan are ordinary. In the Cleveland Clinic cohort of 122,007 patients reported by Mandsager in JAMA Network Open, the least fit fifth had a mortality risk 5 times that of the fittest, and in the PURE study of 139,691 people in the Lancet, each 5 kilogram fall in grip strength raised all-cause mortality by 16%. Fees appear further down this page.

What is a biological age test and what does it measure

A biological age test estimates how old your body is compared with the calendar, usually from a blood or saliva sample. The best-known are epigenetic clocks, which read chemical marks on DNA at hundreds of sites and were first described by Horvath in Genome Biology in 2013. Second-generation clocks such as GrimAge, reported by Lu and colleagues in 2019, were trained on mortality rather than on age, and they predict death and disease better than the calendar does.

The limit is precision and action. A 2022 analysis by Higgins-Chen and colleagues in Nature Aging showed that repeating the same clock on the same sample could shift the result by several years, which is why a consumer kit saying you are 41 in a 46-year-old body is a range, not a fact. No trial has shown that acting on a clock changes an outcome. The CALERIE calorie restriction trial slowed the DunedinPACE clock by 2 to 3% over 2 years, and that is the strongest evidence that exists.

Which 5 routine measures predict lifespan better

Cardiorespiratory fitness is the first, and it is measured rather than estimated. In the Cleveland Clinic cohort of 122,007 patients, the least fit fifth carried a mortality risk around 5 times that of the fittest, an effect larger than smoking or diabetes, with no ceiling at the top. VO2 max is measured on a bike or treadmill with a mask, and the Athlete Screen includes it. Grip strength is the second, because the PURE study of 139,691 people found each 5 kilogram fall raised mortality by 16%.

Apolipoprotein B is the third, since it counts the particles that cause atherosclerosis and captured the lipid risk of heart attack in 389,529 UK Biobank participants in the analysis by Marston in JAMA Cardiology. HbA1c is the fourth, because an HbA1c of 42 to 47 mmol/mol carries a risk of diabetes that the Diabetes Prevention Program cut by 58%. Blood pressure is the fifth, and the SPRINT trial in the NEJM cut cardiovascular events by 25% by treating to a target of 120 rather than 140. None of these needs a clock to interpret.

Send us your results and a doctor will say which of the 5 need work or email team@thewellnesslondon.com.

Which longevity tests should you not buy

Telomere length is the first. It varies by cell type, by laboratory and by day, the test-retest reliability is poor, and no guideline uses it. A consumer epigenetic clock bought to be repeated every 6 months is the second, since the noise between 2 tests is often larger than any change a person could make in that time. An NAD level in blood is the third, because it does not reflect NAD inside cells, no reference range exists, and it is sold to justify an infusion.

A consumer genetic risk kit is the fourth, since polygenic risk scores add little to an ApoB, an HbA1c and a blood pressure that have already been measured, and a pharmacogenomic panel has a handful of real uses a doctor orders when they arise. A senolytic or a peptide sold on the back of any of these tests is the fifth, and none has a human trial showing it extends life.

How is a biological age assessment done at The Wellness

You are assessed first. History, family history, exercise, sleep and examination decide what is measured, and your allocated doctor specifies the list. Blood collection is arranged at an accredited partner laboratory a short walk from the clinic, covering over 100 markers on the Complete Biomarker Assessment, including ApoB, lipoprotein little a, HbA1c, fasting insulin, high-sensitivity CRP, ferritin, thyroid and the liver markers for a FIB-4 score. Where fitness is the question, cardiopulmonary exercise testing is arranged at a specialist centre and reported by a consultant.

Imaging is added where age and history justify it, arranged at the London centre best equipped for each study and reported by a consultant radiologist, with a CT coronary calcium score from 45 and a DEXA for bone and body composition. A review appointment with the doctor who assessed you reads everything together and sets 3 or 4 numbers to move over the next 12 months. The 12-month programmes then repeat the markers quarterly, because the trend is the result, and a single test from a different provider each year measures nothing.

What does a biological age test cost in London

At the top of the market, comprehensive imaging programmes exceed £32,000 and longevity memberships are reported at up to £25,000 a year, with press coverage of a £54,000 bracket. Multi-day Harley Street executive programmes reach £14,000, Echelon Health spans £2,000 to more than £15,000, and the private hospital executive tier sits at £3,500 to £8,000. Bought separately, a cardiopulmonary exercise test with a consultant, a 100-marker panel, a DEXA at £415 and a private GP to interpret them together already pass £2,000 across 4 providers and 4 waits.

Beneath that sits a tier defined by its conditions rather than its price, postal kits that return a single age number with no examination, no doctor and no plan, and memberships built around infusions and supplements rather than measurement. At The Wellness the price includes the assessment, the markers, the fitness test where indicated, the review and the plan. All figures are from prices.

Why The Wellness is the best biological age assessment in London

The numbers measured here are the ones with mortality data behind them, and that is countable. Fitness, grip strength, ApoB, HbA1c and blood pressure each carry a cohort of over 100,000 people or a randomised trial, and each can be moved in 12 months. An epigenetic clock is offered when a person wants one, with the Higgins-Chen noise data explained first, and it is never the number the plan is built on.

Every result is read by the doctor who assessed you, in a review appointment, against every other result, and the 12-month programmes repeat the markers quarterly so that an ApoB falling from 1.3 to 0.8 or a VO2 max rising 4 points is seen rather than guessed. That is why the plan is 3 or 4 targets rather than a page of 40 supplements, and why the trend over 3 years matters more than any single age.

We also say what does not work. No infusion, peptide or senolytic is sold here, a telomere test is refused at any price, and a 34-year-old with an ApoB of 0.7 and a VO2 max in the top fifth is told they are already doing the things that matter and to come back at 40. Places on the 12-month programmes are limited by the number of people one doctor can hold properly, so they begin with an allocation call.

Frequently asked questions

What is the best biological age test in London

The best biological age assessment in London measures the things with mortality data behind them, cardiorespiratory fitness, grip strength, apolipoprotein B, HbA1c and blood pressure, and reads them with over 100 markers in a review appointment with the doctor who assessed you. At The Wellness the Complete Biomarker Assessment is from £1,400 and the Athlete Screen with fitness testing is from £1,695.

Are epigenetic age tests accurate

Partly. Second-generation clocks such as GrimAge predict mortality better than chronological age, but repeating the same clock can shift the result by several years, and no trial shows that acting on a clock changes when anyone dies. At The Wellness a clock is offered on request with that explained, and the plan is built on measured markers within the Complete Biomarker Assessment from £1,400.

How much does a biological age test cost in London

A biological age assessment sits inside longevity memberships of up to £25,000 a year, executive programmes of £3,500 to £8,000 at private hospitals and more than £32,000 at the top of the market, and assembling a fitness test, a 100-marker panel and a DEXA yourself passes £2,000 across 4 providers. At The Wellness the Complete Biomarker Assessment is from £1,400 and the Continuous Care Programme from £24,995.

What actually predicts how long I will live

Cardiorespiratory fitness, where the least fit fifth had 5 times the mortality of the fittest in 122,007 patients, grip strength, apolipoprotein B, HbA1c and blood pressure. Each is measured, each has a cohort or trial behind it, and each can be moved in 12 months. At The Wellness those are measured within the Athlete Screen from £1,695 and the Complete Biomarker Assessment from £1,400.

Is a telomere test worth it

No. Telomere length varies by cell type, laboratory and day, its test-retest reliability is poor and no guideline uses it. The Wellness does not sell it at any price and measures the markers that predict lifespan instead, within the Complete Biomarker Assessment from £1,400.

Do you take the blood and do the tests at the clinic

No. You are assessed at the clinic, blood collection is arranged separately at an accredited partner laboratory, fitness testing and any imaging are arranged at specialist London centres and reported by consultants, and a review appointment goes through everything. The Complete Biomarker Assessment is from £1,400 and the Executive Health Programme from £11,995.

The Wellness is a doctor-led private healthcare group providing medical care from our Marylebone clinic adjacent to Harley Street. All doctors are GMC-registered. Imaging is arranged at specialist centres and reported by consultant radiologists, and blood analysis is performed by accredited laboratories. Chest pain on exertion, fainting during exercise or sudden breathlessness need a doctor before any fitness test, not after it. In an emergency call 999.
