Who should have a low-dose CT of the lungs

The trials answer this precisely. NELSON recruited current and former smokers aged 50 to 74 who had smoked more than 15 cigarettes a day for 25 years or more than 10 a day for 30 years, and stopped no more than 10 years earlier. NLST recruited 55 to 74-year-olds with 30 pack-years. In those groups the scan cut lung cancer deaths by a fifth to a third, because it finds tumours at stage 1 when 3 in 4 are curable rather than at stage 4.

In England the NHS Lung Cancer Screening Programme, recommended by the UK National Screening Committee in 2022 and rolling out to full coverage by 2029, invites everyone aged 55 to 74 who is registered with a GP and has ever smoked to a lung health check, and offers a low-dose CT to those whose risk score qualifies. If you are in that group, take the NHS scan. It is the same test and it is free.

Who should not have one

A never-smoker under 60 with no family history and no asbestos or radon exposure should not, and we do not sell it to them. Around 1 in 4 screening scans show a nodule, and in NLST 96% of those were not cancer, yet each needed a repeat scan at 3 or 12 months and some needed a biopsy. The British Thoracic Society nodule guideline exists because the follow-up burden is real, and creating it in someone whose pre-test risk is a fraction of 1% is harm without benefit.

Around 15% of UK lung cancers occur in people who have never smoked, and that figure is the reason people ask. It does not change the answer, because no trial has shown that scanning never-smokers saves lives, and the Taiwanese TALENT study that scanned never-smoking women with a family history found many cancers but has not yet shown a mortality benefit. A never-smoker with a first-degree relative diagnosed under 60, or with asbestos exposure, is offered the conversation and the choice.

Why a chest X-ray is not screening

The PLCO trial in JAMA in 2011 randomised 154,901 people to annual chest X-ray or usual care and found no reduction in lung cancer deaths at all. An X-ray misses tumours behind the heart, the ribs and the diaphragm, and by the time a cancer is visible it is usually past the stage where surgery cures it. A chest X-ray as a screening test on an executive package costs money and finds nothing, and it appears on many.

Low-dose CT is different because it is a thin-slice scan of the whole chest at around 1.5 millisieverts, roughly 6 months of natural background radiation in the UK, and it sees a nodule of 4 millimetres. Standard CT with contrast is not screening either, since it delivers 5 to 7 millisieverts and adds nothing for nodule detection. The scan is arranged at the London centre best equipped for it, performed by a radiographer and reported by a consultant thoracic radiologist using the British Thoracic Society nodule protocol.

How is lung screening arranged at The Wellness

You are assessed first. Smoking history in pack-years, years since stopping, family history, occupational exposure, breathlessness and cough decide whether the scan is justified, and whether the NHS programme already covers you.
