An executive body scan at The Wellness is a set of targeted studies chosen after you are assessed. The core for most people aged 40 to 65 is a CT coronary artery calcium score, ultrasound of the carotid arteries, an abdominal ultrasound covering liver, kidneys, gallbladder, spleen and aorta, and a DEXA scan for bone density and body composition. Each is performed by an experienced sonographer or radiographer and reported by a consultant radiologist.

The list changes with your history. A 55-year-old who smoked 20 a day for 25 years is offered low-dose CT of the lungs because the NELSON trial and the NHS Targeted Lung Health Check programme both support it in that group. What the scan is not is a whole-body MRI, which the 12-study review found improved no outcome, and which we arrange only when a specific question justifies it.

Which scans are worth having at 40, 50 and 60

At 40 the highest-yield study is the CT coronary artery calcium score, a 10-minute scan at around 1 millisievert. In the MESA cohort of 6,722 adults, a score of 0 carried a coronary event rate near 1 per 1,000 person-years and scores over 300 raised the hazard around 10-fold. That number often decides whether a 42-year-old with an LDL of 3.9 needs a statin now or in 10 years.

At 50 the abdominal ultrasound earns its place because 1 in 4 adults has fatty liver disease according to Younossi and colleagues, and most do not know. Ultrasound sees fat from around 20% liver involvement, and paired with a FIB-4 score from the bloods it sorts the many with simple fat from the few who need a FibroScan and a hepatologist. NHS bowel screening starts at 50 too, and we tell everyone to complete the kit the day it arrives.

At 60 add DEXA, because 1 in 2 women and 1 in 5 men over 50 will break a bone through osteoporosis according to the Royal Osteoporosis Society. Men reaching 65 have the aorta measured once, since the MASS trial of 67,800 men showed a 42% reduction in aneurysm deaths over 4 years from a single ultrasound.

Which scans should you not buy, even from us

Thyroid ultrasound in someone with no lump and normal thyroid bloods is the clearest example. When South Korea began screening thyroids with ultrasound, diagnoses rose 15-fold between 1993 and 2011 while deaths from thyroid cancer did not change, as Ahn, Kim and Welch reported in the NEJM in 2014. The scan finds nodules in up to 68% of adults and almost none of them matter. We image a thyroid only for a lump, a symptom or an abnormal result.

Whole-body MRI for a healthy 35-year-old with no family history falls into the same category. Around a third of people in the published series had findings that needed follow-up, mostly benign cysts, and each follow-up carries a cost and a wait. A brain MRI in someone without headaches, seizures or focal symptoms finds an incidental abnormality in around 2.7% of people according to the meta-analysis by Morris and colleagues in the BMJ, and almost all lead nowhere useful.

The same logic applies to a PSA test bundled into a scan package for a 50-year-old with no family history. In June 2026 the government accepted the UK National Screening Committee's recommendation for targeted PSA screening only in men aged 45 to 61 with a BRCA2 variant and a family history, because whole-population screening does more harm than good. We discuss PSA as an informed choice and never add it silently.

How is the scan arranged and who reports it

You are assessed first. History, family history and examination decide what is imaged and why, and your allocated doctor specifies the studies.
