The scan takes under 10 minutes, needs no injection, and delivers around 1 millisievert, roughly 4 months of natural background radiation in the UK. It is performed by a radiographer and reported by a consultant.

What does the calcium score number actually mean
A score of 0 means no calcified plaque was seen, which in MESA carried a coronary event rate near 1 per 1,000 person-years, and Nasir and colleagues showed in JACC in 2015 that over 4 in 10 adults who qualified for a statin on risk calculators alone had a score of 0 and an event rate low enough to defer treatment. A score of 1 to 99 means plaque is present and the treatment conversation starts, 100 to 399 usually means treatment, and 400 or more means treatment plus a cardiologist.
The number means different things at different ages. A score of 50 in a 45-year-old is a stronger signal than the same score in a 70-year-old, which is why the result is reported as a percentile for age and sex as well as a raw score. Min and colleagues in JACC in 2010 put the warranty period of a score of 0 at around 4 years, after which the scan is repeated if risk factors persist.

When is a score of 0 not enough
Under 45, a score of 0 is reassuring but not conclusive, because early plaque is soft and the scan cannot see it. That is why the scan is not sold here to a symptomless 35-year-old with no family history, and why a 42-year-old whose father had a heart attack at 50 is offered the blood markers first. Lipoprotein little a is inherited and raised in 1 in 5 people, and the Copenhagen study by Kamstrup and Nordestgaard found the highest levels doubled heart attack risk regardless of the calcium score.
Symptoms change the test. Chest tightness on exertion or breathlessness out of proportion to effort are not screening questions, and NICE guideline CG95 directs them to CT coronary angiography with contrast, which sees soft plaque and narrowing the calcium score misses. The SCOT-HEART trial in the NEJM in 2018 cut death or heart attack by 41% over 5 years when CT angiography guided the care of people with chest pain.
Which heart tests should you not buy
A resting ECG as a screening test in someone with no symptoms is the clearest example. The US Preventive Services Task Force advises against it in low-risk adults because false positives lead to angiograms, and it cannot see plaque. A treadmill stress ECG in a symptomless adult performs little better. Both appear on executive packages because they are cheap to run.
Carotid intima-media thickness sold alone is the second, since it measures wall thickness rather than plaque and the European Society of Cardiology dropped it from routine risk assessment in 2016. Carotid ultrasound for plaque still earns its place when the calcium score is borderline, which is why the Heart Health programme includes it. A repeat calcium score within 4 years of a score of 0 is the third.
Who should have a calcium score in London
