How the programme runs

‍ ‍

Every programme begins with an allocation call rather than a booking form, because the assessment is designed before anything is ordered. You are then allocated a doctor who takes a full history, family history and examination, and specifies exactly what should be tested and why. Nothing is booked until that is settled, which is what separates a programme built around one person from a package sold to a population.

We then run everything on your behalf. Imaging goes to the London centre chosen for your specific question and the equipment it holds, reported by consultant radiologists. Bloods go to accredited laboratories. Specialist opinions are arranged through the network alongside us. Appointments are scheduled around your diary, chased, and confirmed to you rather than left in a portal.

The biomarkers that change decisions

Around 80 percent of what an executive programme finds comes from blood, so composition matters far more than length. Apolipoprotein B counts the particles that actually enter artery walls and identifies people whose standard cholesterol reads fine while their particle burden does not, which describes a large share of this population. Lipoprotein little a is inherited, unaffected by diet or training, raised in 1 in 5 people and established as causal by Kamstrup and Nordestgaard in the Copenhagen population studies. HbA1c finds the prediabetic decade while it is still reversible, and the Diabetes Prevention Program reported a 58 percent reduction in progression to diabetes from lifestyle intervention alone, the largest reversible gain available to anyone in their 40s. Around those sit liver enzymes, kidney function, thyroid, full blood count, inflammatory markers, ferritin and iron studies, B12, folate and vitamin D, then morning testosterone where symptoms suggest it, PSA after the informed choice conversation, and hormonal assessment through the perimenopausal years. Marker count beyond that is decoration.

Imaging, aimed and sent to the right place

Untargeted whole-body scanning of people without symptoms has been reviewed across 12 studies and 5,373 participants with no demonstrated improvement in outcomes, while incidental findings are common and generate further scans, biopsies and worry. The Royal College of Radiologists has cautioned about this for years. Aimed imaging is a different proposition entirely. An abdominal and aortic assessment in a man over 65 never screened for aneurysm. A liver assessment where the enzymes and metabolic picture warrant one. Coronary artery calcium scoring in the intermediate risk band, where Detrano and colleagues in the Multi-Ethnic Study of Atherosclerosis showed it reclassifies risk better than any blood marker and where a score of 0 can justify holding off on lifelong medication. Multiparametric prostate MRI where a PSA result requires it.

Owning no scanner is the single most useful thing about how this works. There is no equipment here to keep busy and no reason to recommend the modality we happen to have, so each study goes to the centre best equipped for the question and is reported by consultant radiologists who read that anatomy every day.
