Which inherited conditions are worth testing for
Familial hypercholesterolaemia first, because it is the commonest serious inherited condition and the most treatable. An LDL above 4.9 mmol/L in an adult, or above 4.0 in a child, with a parent or sibling who had a heart attack under 60, meets the NICE guideline CG71 criteria, and a confirmed variant triggers cascade testing of every first-degree relative, each of whom has a 50% chance of carrying it. Treatment from childhood restores a normal life expectancy.
Lipoprotein little a is second, measured once because it is fixed by genes, and a raised level in one parent means each child should be tested once in their 20s. BRCA1 and BRCA2 are third. NICE guideline CG164 and the NHS genomic test directory offer testing where the family history gives a 10% or higher chance of a variant, calculated with the CanRisk tool, or where there is Ashkenazi ancestry with any breast or ovarian cancer in the family. A woman with a mother and a sister with breast cancer under 50 qualifies, as does a man with a father who had bowel cancer at 45 and an aunt with womb cancer, or an adult with an LDL of 5.2 and a brother who had a stent at 48.
If you qualify, the NHS test is the right test, with genetic counselling before and after, and we say so. What a private programme adds is speed, a single doctor holding the whole family's results, and the management afterwards, which the NHS provides through 3 or 4 separate services with separate waits. What a family programme actually does
Each family member has their own doctor and their own record, and the family history is taken once and held for all of them. Inherited risk is a family fact rather than an individual one. A raised lipoprotein little a in a father changes what is measured in his daughter, a BRCA2 variant in a mother changes her son's prostate screening under the June 2026 UK National Screening Committee decision, and a Lynch result in an uncle changes the age at which a niece starts colonoscopy.
