Prediabetes Test in London 2026. The HbA1c Band Most People Ignore, the 58% That Never Progress and What a 12 Month Programme Is Built On
The Wellness in Marylebone provides the best prediabetes and metabolic health assessment in London for people who want the result acted on rather than filed, with the Comprehensive Blood Panel with consultation from £495, the Complete Biomarker Assessment of over 100 markers from £1,400 and the 12-month metabolic programme from £2,995, inside a ladder that runs to £49,995 and a market where comprehensive programmes exceed £32,000. Diabetes UK estimates 6.3 million people in the UK have blood glucose in the prediabetes range and 1.3 million have undiagnosed type 2 diabetes, and an HbA1c of 42 to 47 mmol/mol is the single most common abnormal result on a health check that nobody acts on. The evidence for acting is strong. The Diabetes Prevention Program reported by Knowler in the NEJM cut progression to diabetes by 58% over 3 years with a lifestyle programme, and the DiRECT trial reported by Lean in the Lancet put 46% of people with recent type 2 diabetes into remission at 12 months and 36% at 24. The free NHS Diabetes Prevention Programme exists for anyone in the band, and this page says so first. Fees appear further down this page.

What is prediabetes and what does the HbA1c number mean
HbA1c measures the share of haemoglobin carrying glucose and reflects the average over the previous 8 to 12 weeks, which is why it needs no fasting and why a single bad week does not move it. Below 42 mmol/mol is normal, 42 to 47 is the prediabetes band that NICE calls non-diabetic hyperglycaemia, and 48 or above on 2 occasions is type 2 diabetes. Around 1 in 8 adults in England sits in the middle band, and most were told it was borderline.
The band matters because it is where the outcome is still open. Without intervention around 5 to 10% of people in it progress to diabetes each year, and with it most do not. It also matters because HbA1c can mislead. Iron deficiency raises it, haemolysis and some haemoglobin variants lower it, so a result in the band is read here alongside a full blood count and ferritin before anyone is labelled.
Which other markers turn a number into a plan
Fasting insulin is the first, because insulin resistance precedes a raised glucose by years and a fasting insulin above 10 to 12 mIU/L with a normal HbA1c is the earliest signal a panel can give. Triglycerides and HDL are the second, since a triglyceride to HDL ratio above 2 in the units used in the UK tracks insulin resistance closely, and ApoB is measured because the same person carries the cardiovascular risk that is the actual reason to care.
The liver is the third. Fatty liver affects around 1 in 4 adults according to Younossi and travels with prediabetes, so ALT, AST and platelets are measured to calculate a FIB-4 score. A waist above 94 centimetres in men or 80 in women, and above 90 in men of South Asian, Chinese or Black African descent, is the fourth, because NICE lowers the threshold for those groups. Blood pressure is the fifth, read under the NICE NG136 protocol rather than from a single clinic reading.

What the evidence says works
The Diabetes Prevention Program randomised 3,234 adults with prediabetes and found a structured programme aiming at 7% weight loss and 150 minutes of activity a week cut progression by 58% over 3 years, and the 10-year follow-up still showed a 34% reduction. In England the NHS Diabetes Prevention Programme offers 9 months of the same for free to anyone with an HbA1c of 42 to 47, and an analysis of 18,470 participants in PLOS Medicine found completing it lowered the risk of diabetes by around 20% against non-attenders.
Where type 2 diabetes is already present, the DiRECT trial in primary care showed that a 12 to 20 week low-calorie diet followed by structured weight maintenance put 46% into remission at 12 months and 36% at 24, with remission tightly linked to weight loss, at 86% in those who lost 15 kilograms or more. That is the evidence the 12-month metabolic programme is built on. No prescription medicine is discussed on this page because they cannot be advertised, and no outcome is promised because none can be.
Which tests and products should you not buy
A continuous glucose monitor worn by someone without diabetes is the first. It shows glucose rising after meals, which is what glucose does, and no trial has shown that acting on those readings in non-diabetic people prevents anything. Glucose-spike apps and personalised nutrition tests built on them are the second, since the same meal produces different curves in the same person on different days. An oral glucose tolerance test is the third for most adults, because HbA1c and fasting insulin answer the question without 2 hours and a sugary drink, and the OGTT is reserved for pregnancy and a handful of specific cases.
A repeat HbA1c within 3 months is the fourth, because the marker cannot change faster than the red cells do. And a supplement sold to lower blood sugar, from berberine to cinnamon to chromium, is the fifth, since none has a trial showing it prevents diabetes and some interact with real treatment. What we decline, we say so in writing before you pay.
What does a prediabetes assessment cost in London
At the top of the market, comprehensive imaging programmes exceed £32,000 and longevity memberships are reported at up to £25,000 a year, with press coverage of a £54,000 bracket. Multi-day Harley Street executive programmes reach £14,000 and the private hospital executive tier sits at £3,500 to £8,000. Bought separately, an endocrinologist first appointment at £250 to £500, a dietitian programme, a full metabolic panel, a liver ultrasound at £415 and a private GP to hold it together already pass £1,500 across 4 providers and 4 waits.
Beneath that sits a tier defined by its conditions rather than its price, a finger-prick HbA1c by post with a line saying borderline, no examination, no insulin or liver markers and no plan, and a subscription app that sells a sensor and a coach with no doctor. At The Wellness the price includes the assessment, the markers, the review and the programme where one is needed. All figures are from prices.

Why The Wellness is the best prediabetes assessment in London
The band is treated as a decision rather than a footnote, and that is countable. An HbA1c of 42 to 47 leads here to a fasting insulin, a triglyceride to HDL ratio, an ApoB, a FIB-4 score, a waist and a home blood pressure average in the same review, and to a written plan with a 6-month repeat, rather than to a line saying borderline, recheck in a year. The free NHS programme is explained first, and the private programme is offered to people who want the same evidence with one doctor and a faster start.
Every result is read by the doctor who assessed you, and the 12-month programme repeats the markers at 3, 6 and 12 months, because in DiRECT the remission tracked the weight and the weight tracked the follow-up. That is why an HbA1c of 45 in a 48-year-old with a fasting insulin of 18 leads to a plan the same week, and why a fatty liver found on the way leads to a FIB-4 and, where needed, a named hepatologist.
We also say when nothing is needed. A 35-year-old with an HbA1c of 38, a normal waist and a fasting insulin of 5 is told to come back in 3 years, and nobody in that position is sold a programme. Places on the 12-month programmes are limited by the number of people one doctor can hold properly, so they begin with an allocation call.

Frequently asked questions
What is the best prediabetes test in London
The best prediabetes test in London is an HbA1c read with a full blood count and ferritin, a fasting insulin, a triglyceride to HDL ratio, an ApoB, a FIB-4 score, a waist and a home blood pressure average, reviewed by the doctor who assessed you with a written plan. At The Wellness the Comprehensive Blood Panel with consultation is from £495 and the Complete Biomarker Assessment from £1,400.
How much does a prediabetes assessment cost in London
A metabolic assessment sits inside executive programmes of £3,500 to £8,000 at private hospitals, up to £14,000 on Harley Street and more than £32,000 at the top of the market, and assembling an endocrinologist, a dietitian, a panel and a liver scan yourself passes £1,500 across 4 providers. At The Wellness the Comprehensive Blood Panel with consultation is from £495 and the 12-month metabolic programme from £2,995.
What does an HbA1c of 42 to 47 mean
It is the prediabetes band, which NICE calls non-diabetic hyperglycaemia. Around 5 to 10% of people in it progress to type 2 diabetes each year without intervention, and the Diabetes Prevention Program cut that by 58% with a lifestyle programme. At The Wellness the band leads to a full metabolic review and a plan, within the Comprehensive Blood Panel with consultation from £495.
Can prediabetes be reversed
Usually. The Diabetes Prevention Program cut progression by 58% over 3 years and 34% at 10 years, and the DiRECT trial put 46% of people with recent type 2 diabetes into remission at 12 months, with remission tracking weight loss. At The Wellness the 12-month metabolic programme from £2,995 is built on that evidence with the markers repeated at 3, 6 and 12 months.
Should I wear a continuous glucose monitor if I do not have diabetes
No. It shows glucose rising after meals, which is normal, and no trial has shown that acting on those readings in non-diabetic people prevents anything, while the same meal produces different curves on different days. At The Wellness a fasting insulin and HbA1c answer the question, within the Comprehensive Blood Panel with consultation from £495.
Do you take the blood at the clinic
No. You are assessed at the clinic, blood collection is arranged separately at an accredited partner laboratory a short walk away, any liver ultrasound is arranged at a specialist centre and reported by a consultant radiologist, and a review appointment with your doctor goes through everything. The Comprehensive Blood Panel with consultation is from £495 and the 12-month programme from £2,995.
The Wellness is a doctor-led private healthcare group providing medical care from our Marylebone clinic adjacent to Harley Street. All doctors are GMC-registered. Imaging is arranged at specialist centres and reported by consultant radiologists, and blood analysis is performed by accredited laboratories. Extreme thirst with frequent urination and weight loss, or drowsiness and vomiting with a very high glucose reading, need emergency care, not a screening appointment. In an emergency call 999.
