What is an immigration medical and who needs one?
An immigration medical is a health assessment required by UK Visas and Immigration (UKVI) as part of certain visa applications. The purpose is to confirm that the applicant does not have a health condition that poses a public health risk, particularly active pulmonary tuberculosis, and to assess fitness for entry or settlement.
Under current UKVI rules, a TB test certificate is required for visa applicants who are nationals of, or have recently lived in, any of the countries on the Home Office's designated list. This list includes over 100 countries across Africa, Asia, South America, and parts of Eastern Europe. The TB test must be conducted before the visa application is submitted and the certificate must be included with the application.
Applicants who may need an immigration medical include those applying for a settlement visa (spouse, partner, family reunion), Indefinite Leave to Remain (ILR), student visas from designated countries, work visas from designated countries, asylum seekers and those with pending human rights claims, and anyone whose visa application requires evidence of health status.

TB test for UK visa applications
The TB test is the most common immigration health requirement. UK Visas and Immigration requires applicants from designated countries to provide a certificate confirming they are free from active pulmonary tuberculosis before their visa can be processed.
What the TB test involves. The standard screening for immigration purposes is a chest X-ray, which is reviewed by a doctor to check for signs of active pulmonary TB. If the chest X-ray is normal, a TB certificate is issued confirming the applicant is clear. If the chest X-ray shows abnormalities suggestive of TB, further testing (sputum samples for culture and PCR) may be required. The Online GP arranges same-day chest X-rays at partner imaging facilities in central London, with results available within hours.
TB certificate requirements. The certificate must state the applicant's name, date of birth, and passport number. It must confirm that the applicant has been screened for active pulmonary TB and found to be clear. It must include the date of the test, the doctor's name, GMC registration number, and clinic details. The certificate is valid for six months from the date of issue.
Which countries require a TB test for UK visa? The Home Office maintains a list of over 100 countries. Major countries on the list include India, Pakistan, Bangladesh, Nigeria, Ghana, Kenya, South Africa, China, the Philippines, Thailand, Vietnam, Indonesia, Egypt, Sudan, Somalia, Ethiopia, and many others across Africa, Asia, and South America. ILR medical assessment (Indefinite Leave to Remain)
Applicants for Indefinite Leave to Remain (ILR) may be required to demonstrate that they meet the health requirements of the Immigration Rules. While not all ILR applications require a medical, some routes and circumstances do, particularly where a previous visa was granted subject to health conditions, or where UKVI specifically requests a medical assessment.
The ILR medical assessment at The Online GP includes a general health assessment by a GMC-registered doctor, review of vaccination history, TB screening if applicable, and a written medical report confirming the applicant's health status in the format required by UKVI.
Medical reports for immigration and asylum cases
Immigration cases beyond standard visa applications may require detailed medical reports. These are substantively different from a TB certificate, requiring thorough clinical assessment and professionally structured medical documentation.
Medical reports for asylum claims and human rights cases. Where an applicant's medical condition is relevant to their claim, a medical report documenting injuries, scars, or psychological conditions consistent with the claimant's account may be required. In appropriate cases, reports may reference the Istanbul Protocol (the UN Manual on Effective Investigation and Documentation of Torture and Other Cruel, Inhuman or Degrading Treatment or Punishment). These reports must be factual, objective, and clearly distinguish between the doctor's clinical findings and their professional opinion.
Medical reports for immigration bail. Where a detainee's health is relevant to bail applications or challenges to removal, a medical report documenting current health status and the potential impact of continued detention or removal on the applicant's health.
Medical fitness-to-travel assessments. Where immigration removal requires medical clearance, or where an applicant needs to demonstrate that travel would pose a health risk.
Medical reports for visa extensions and appeals. Supporting evidence for applications to extend a visa on health grounds, or for appeals against visa refusals where health is a relevant factor.
All immigration medical reports are prepared by GMC-registered doctors and comply with the standards expected by the Home Office, immigration judges, and the First-tier Tribunal (Immigration and Asylum Chamber).

3. Assessment. A GMC-registered doctor conducts the medical assessment at our Marylebone clinic. For TB screening, a chest X-ray is arranged at a partner imaging facility nearby, with results returned within hours.
4. Certificate or report issued. TB certificates are typically issued same-day. Standard immigration medicals and ILR assessments are issued same-day. Detailed immigration reports for asylum, bail, or appeal cases take three to five working days.

Important information
