What a longevity assessment is actually trying to do
The useful definition is simpler than the marketing. A longevity assessment should identify modifiable risks that may affect how well you live over the next ten, twenty and thirty years, establish a baseline while you are well, and give you something measurable to improve.
That means cardiovascular risk before someone develops cardiovascular disease. Metabolic dysfunction before diabetes develops. Nutritional deficiency before it becomes symptomatic. Blood pressure, body composition and bone health before the consequences of deterioration become difficult to reverse. It also means understanding inherited risks that do not necessarily announce themselves through symptoms.
The objective is not to prove that you are biologically younger than your passport says. It is to find the things that can reasonably be measured, interpreted and acted on.
The biomarkers worth knowing before you pay for the exotic ones
A sophisticated-looking report can contain hundreds of measurements while missing some of the measurements that matter most.
For cardiovascular risk, a standard cholesterol panel is useful but incomplete. ApoB helps quantify the number of atherogenic particles circulating in the blood, while lipoprotein(a), or Lp(a), identifies an important largely genetically determined cardiovascular risk that may not be obvious from LDL cholesterol alone. Blood pressure, smoking history, family history and the rest of the clinical picture still matter just as much. No single biomarker replaces the assessment.
Metabolic health deserves the same attention. HbA1c gives a longer-term picture of glucose exposure, while fasting glucose and fasting insulin can add useful information in the right person about how the body is handling glucose before overt diabetes develops.
Then come the measurements that explain how you actually feel. Full blood count and iron studies. Thyroid function. B12 and folate. Vitamin D. Magnesium. Sex-specific hormonal markers where clinically relevant. Kidney and liver function. Inflammatory markers.
A good longevity panel measures enough to find useful patterns without pretending that every measurable molecule has been proven to predict lifespan.

The 5 questions to ask any longevity clinic before paying
Ask what decisions will change because of the tests being performed. If nobody can answer that, you may be buying information rather than medicine.
Ask who interprets everything together. A cardiology result, hormone panel and scan reported separately are not the same thing as one physician reading the entire picture.
Ask whether ApoB and Lp(a) are measured. A longevity programme with a very long biomarker list but no serious cardiovascular risk assessment has its priorities backwards.
Ask which treatments are established medical care and which are experimental or optimisation-focused. The distinction should be stated clearly.

Do I need a whole-body MRI for longevity screening
Not automatically. MRI can be extremely useful where a person's history, risk or symptoms justify it, but screening every healthy person with every available imaging modality can also generate incidental findings and additional investigations. Imaging should be selected against your clinical situation.
What is ApoB
ApoB is a marker related to the number of atherogenic lipoprotein particles capable of contributing to plaque formation in arteries. It can add information beyond a standard LDL cholesterol result when cardiovascular risk is being assessed.
What is Lp(a)
Lipoprotein(a), or Lp(a), is a largely genetically determined cardiovascular risk factor. Because levels are relatively stable throughout life, measuring it can reveal inherited risk that may not be obvious from a conventional cholesterol panel.
