What does a mole check at The Wellness include

A full skin examination takes around 30 minutes and covers the whole surface, including the scalp, between the toes, the soles and the nails, because melanoma appears in places nobody looks. Every pigmented lesion is examined with a dermatoscope, a handheld magnifier with polarised light that shows the pigment network and vessels beneath the surface, and the lesions that matter are photographed so that change can be measured at the next visit.

A lesion that meets the criteria for removal is removed at the clinic by curettage and cautery or by excision, and every specimen goes to histology, because a mole removed without a pathology report is a mole nobody has diagnosed. A lesion that needs a dermatologist, a wide excision or a sentinel node discussion goes to a named consultant dermatologist with the appointment already booked, with the dermoscopic images attached.

What dermoscopy sees that the eye cannot

The ABCDE rule, asymmetry, border irregularity, colour variation, diameter over 6 millimetres and evolution, is what the eye can assess, and it misses early melanoma because early melanoma can be symmetrical, small and one colour. Dermoscopy sees the atypical pigment network, the blue-white veil, irregular dots and streaks and the vascular patterns that separate a melanoma from a benign naevus, a seborrhoeic keratosis or a haemangioma, and in Vestergaard's pooled analysis of 9 studies it roughly halved the number of benign lesions cut out to find one melanoma.

The ugly duckling sign is the other tool. A person's moles usually look like each other, and the one that looks different from the rest, whatever its size, is the one to examine, which is why a full skin check finds melanomas the person never noticed. Nodular melanoma, around 1 in 5 cases, breaks every rule by being symmetrical, firm and often pink, and grows fast, so a new raised lesion that has changed in weeks is examined the same week rather than watched.

Who needs a full skin check and how often

Anyone with a personal or family history of melanoma, more than 50 moles, atypical moles, fair skin that burns, a history of sunbed use or blistering sunburn in childhood, or a job or sport with heavy sun exposure needs a full examination and a baseline set of dermoscopic images. For that group a check every 12 months is reasonable, and in the study by Truong and colleagues in JAMA Dermatology, total body photography in high-risk patients cut unnecessary biopsies while melanoma detection was unchanged.

Someone with olive skin, few moles and no history does not need an annual check, and is told so. A single new or changing lesion needs one appointment, not a programme. Non-melanoma skin cancers are far more common at around 156,000 a year in the UK, and a basal cell carcinoma, the pearly bump that bleeds and never heals, is found and treated at the same examination. Squamous cell carcinoma, the scaly firm lump that grows over months, is the one that needs removal promptly.

Which mole treatments should you not buy

Laser or cryotherapy removal of a pigmented lesion is the first. NICE guideline NG14 is explicit that a pigmented lesion suspected of being melanoma must be excised whole with a margin and sent for histology, and destroying it with laser or freezing removes the evidence and delays the diagnosis. A mole removed for cosmetic reasons without a pathology report is the second, and a clinic that offers removal of a pigmented lesion without histology should not be used.

A smartphone app that grades your mole is the third. The systematic review by Freeman and colleagues in the BMJ in 2020 found the algorithm-based apps had poor and variable accuracy and were not fit to replace examination, and the Medicines and Healthcare products Regulatory Agency has acted against several. A mole mapping service without a dermatoscope is the fourth, because photographs of the surface are the ABCDE rule at a distance. And a full skin check sold to a 30-year-old with 10 moles, dark skin and no history is the fifth, which we decline.

Nothing pigmented is lasered or frozen here, nothing is removed without a pathology report, and the dermoscopic images are held so that a lesion watched in September is compared against itself in March.
