What is an occupational health assessment?

An occupational health assessment is a medical evaluation that examines the relationship between a person's health and their ability to perform their job safely and effectively. Unlike a standard GP consultation that focuses on diagnosis and treatment, an occupational health assessment focuses on functional capacity, how a medical condition affects the ability to carry out specific work duties, and what adjustments or support may be needed.

Occupational health assessments are used across all industries but are particularly important in sectors where health directly affects safety, including transport, construction, healthcare, offshore and maritime, aviation, food production, and emergency services. They are also increasingly common in corporate and professional services for managing sickness absence, supporting return to work, and fulfilling employers' legal obligations under the Health and Safety at Work Act 1974 and the Equality Act 2010.

Pre-employment medical assessments

A pre-employment medical determines whether a prospective employee is medically fit to perform the duties of the role they have been offered. Many employers require pre-employment medicals as a condition of employment, particularly in regulated industries.

Standard pre-employment medical from £295. Includes a health questionnaire review, clinical history, physical examination (blood pressure, BMI, cardiovascular, respiratory, musculoskeletal, neurological screening), urinalysis, and a written fitness-for-role report. Suitable for office-based, professional, and general employment.

Enhanced pre-employment medical from £495. Includes everything in the standard assessment plus audiometry (hearing test), visual acuity testing, lung function testing (spirometry), and blood tests (full blood count, liver function, kidney function, glucose). Required for roles in construction, transport, manufacturing, healthcare, and other sectors with specific health requirements.

Role-specific pre-employment medicals. Some industries have specific medical standards. The Online GP provides assessments to meet the requirements of HSE guidance for various sectors, the Construction (Design and Management) Regulations 2015, offshore medical standards (OGUK/UKOOA), and other regulatory frameworks. Pricing is confirmed on a case-by-case basis.

Reports are provided to the employer (with the employee's written consent) and clearly state whether the individual is fit for the role, fit with adjustments, or unfit. The report does not disclose the specific medical diagnosis to the employer unless the employee consents, in accordance with the Access to Medical Reports Act 1988.

HGV, PSV, and taxi driver medicals (D4 medical)

Drivers of heavy goods vehicles (HGV/LGV), public service vehicles (PSV/PCV), and taxi/private hire vehicles are required to pass a medical assessment to obtain or renew their licence. The medical assessment follows the DVLA's Group 2 medical standards and is documented on the D4 medical examination report form.

D4 driver medical from £150. Includes assessment of vision (visual acuity, visual fields), cardiovascular health, neurological function, diabetes screening, and general fitness to drive. The doctor completes the D4 form, which the driver submits to the DVLA or the relevant licensing authority.

Taxi and private hire medicals from £150. London taxi (black cab) and private hire vehicle (PHV) drivers licensed by Transport for London (TfL) require medical assessments at initial application and at regular intervals. The assessment covers the same standards as the D4 medical.

Management referral health assessments

A management referral occurs when an employer asks for a medical opinion on an employee's health and its impact on their work. This is the most clinically complex type of occupational health assessment and requires a doctor experienced in balancing the employer's operational needs with the employee's medical confidentiality and legal rights.

Common reasons for management referral include long-term or frequent sickness absence, concerns about an employee's fitness to perform their role safely, requests for guidance on reasonable adjustments under the Equality Act 2010, disputes about capability or fitness for work, and mental health concerns affecting performance or attendance.

Management referral assessment from £495. Includes a detailed consultation with the employee (30 to 45 minutes), review of any medical records provided (with the employee's consent), and a written report to the employer addressing the specific questions posed in the referral.

The report will address whether the employee has a medical condition affecting their work, whether the condition is likely to constitute a disability under the Equality Act 2010, what adjustments may help the employee perform their role, an estimated timeline for return to work where applicable, whether any further medical investigation or specialist referral is recommended, and whether the condition is likely to recur.

The report does not disclose the specific diagnosis to the employer unless the employee provides explicit written consent. The employee has the right to see the report before it is sent, under the Access to Medical Reports Act 1988.

Complex management referral (multiple conditions, extensive records review, or contentious cases) from £750.

Return-to-work and fitness-for-work assessments

After a period of absence due to illness, surgery, or injury, some employers require a medical assessment before the employee returns to duties. This is particularly common after extended absence (28+ days), mental health absence, musculoskeletal conditions, post-surgical recovery, and any absence where there is a safety-critical element to the role.

Return-to-work assessment from £295. The doctor assesses the employee's current fitness, reviews any treatment received, and provides a clear written opinion on fitness to return, with or without adjustments. Where adjustments are recommended (phased return, amended duties, reduced hours), these are documented in the report.

Night worker health assessments

Under the Working Time Regulations 1998 (Regulation 7), employers are legally required to offer a free health assessment to any employee who works night shifts (defined as a period of at least seven hours including the hours between midnight and 5am). This is a statutory obligation, not optional.

Night worker health assessment from £150 per employee. Includes a health questionnaire and clinical review focused on the health effects associated with night work, including sleep disorders, cardiovascular risk, metabolic health, and mental health. A written report confirms the employee's fitness for night work.

Executive health screening

For senior executives, board members, and high-value employees, a comprehensive health screen goes beyond standard occupational health to provide a thorough assessment of current health status and future risk.

Executive health screen from £795. Includes an extended GP consultation (45 minutes), comprehensive blood panel (full blood count, liver and kidney function, lipid profile, HbA1c, thyroid function, PSA where appropriate, vitamin D, B12, ferritin, inflammatory markers), resting ECG, blood pressure, BMI and body composition, urinalysis, cardiovascular risk assessment, cancer screening discussion, lifestyle and stress assessment, and a detailed written health report with personalised recommendations. Additional investigations (ultrasound, cardiac stress test, imaging) can be arranged.

Corporate accounts and employer services

The Online GP by The Wellness welcomes corporate clients. We provide occupational health services on an ad hoc or retained basis for employers of all sizes across London.

All occupational health assessments are at the sole discretion of the assessing doctor. The report reflects the doctor's honest clinical findings and professional opinion. Reports cannot be amended to change the clinical opinion.

Employee consent and confidentiality. Medical information is only shared with the employer with the employee's explicit written consent. Under the Access to Medical Reports Act 1988, employees have the right to see the report before it is sent. The report addresses the employer's questions about fitness and adjustments without necessarily disclosing the specific diagnosis.

Acceptance of cases is at the complete discretion of the clinic. Final charges are determined by the assessment requirements.

Frequently asked questions about occupational health assessments

Can an employee refuse an occupational health assessment? An employee can decline, but the employer may be entitled to make decisions about fitness for work based on the information available without the medical input. Employees should be aware that declining an assessment may limit the employer's ability to make adjustments or support a return to work.

Do you provide drug and alcohol testing? Yes. Urine drug screening and alcohol testing can be included as part of a pre-employment or workplace health assessment. Pricing confirmed on request.

Can assessments be done via video? Some assessments (management referrals, return-to-work discussions) can be conducted via video. Pre-employment medicals, D4 driver medicals, and assessments requiring physical examination or testing must be conducted in person.
