What one-day screening should actually mean
A genuine one-day programme means the part requiring your physical presence is organised efficiently. It does not mean pretending every sophisticated test in modern medicine reports within six hours.
Some blood results return quickly. Others take longer. Consultant radiology reports need proper review. Certain specialist tests have processing times that shouldn't be shortened because someone has a flight to catch.
The efficient model compresses attendance, not interpretation. Your history is reviewed before the day, investigations are specified in advance, appointments are sequenced, you attend once for the in-person components, and results feed into one record and one final physician review. That beats an impressive-looking itinerary followed by six unrelated reports landing in six different portals.
The work begins before you arrive
The most valuable hour in a premium executive assessment often happens before you reach the clinic. Previous bloods, imaging, medication, family history and specific concerns are reviewed in advance. A 48-year-old founder whose father had a heart attack at 52 doesn't need the identical programme as a 48-year-old with a strong family history of bowel cancer.
This is where fixed, standardised packages lose efficiency. If the programme is set before the doctor knows who you are, you either buy tests you don't need, or discover halfway through the day that the investigation you did need was never booked.
What the day itself can include
Blood testing comes first, particularly for fasting markers. History and examination establish the clinical context the numbers sit inside. Blood pressure, cardiovascular risk assessment and ECG are done in the same visit. Body composition or DEXA is added where useful. Targeted imaging follows, whether coronary calcium CT, ultrasound, MRI or another study, selected to your actual risk rather than defaulting to the biggest scan available.
If a scan needs fasting, contrast preparation or a specific time window, the rest of the programme moves around it, rather than sending you back on a second day.
What a serious executive blood panel should contain
Executives don't have a different cardiovascular system from anyone else. A premium panel still has to get the fundamentals right: full blood count, kidney and liver function, glucose regulation, lipids, thyroid assessment where appropriate, nutritional markers, plus whatever your personal history justifies.
The cardiovascular layer should go deeper than total cholesterol. ApoB and Lp(a) add real information about atherogenic and inherited risk. Metabolic assessment should look beyond body weight. HbA1c, glucose and, in selected patients, fasting insulin build the fuller picture. Hormonal testing should be used when clinically relevant, not because it markets well.
Final interpretation should distinguish a genuinely important abnormality from a result that's fractionally outside a lab reference range with no real significance, which matters more, not less, as the biomarker count rises. Why more expensive doesn't mean more comprehensive
Price is a poor proxy for clinical completeness, because programmes charge for different things. Conventional assessments start around £1,129. Imaging-heavy platinum programmes reach around £14,000. Full itinerary-plus-membership models start at £15,000, then move to £54,000 a year. Ultra-premium longevity memberships run to $100,000 a year internationally.
The price climbs because the product changes. A one-off assessment becomes advanced diagnostics. Advanced diagnostics become twelve months of physician access, repeated testing, performance medicine, nutrition and concierge support. Those may all be valuable. They just shouldn't be confused with the screening itself.

The mistake that costs the most time: fragmented care
One clinic does the bloods. Another does the MRI. A cardiology centre reports a CT. Your GP receives none of it. You end up downloading five PDFs and emailing them to someone new.
