Why Breast Lumps Need Assessment
Most breast lumps are benign. The most common causes include fibroadenomas (smooth, mobile, solid lumps most common in women aged 15-35), breast cysts (fluid-filled sacs that can appear suddenly and feel round and smooth), fibrocystic changes (lumpy, sometimes tender breast tissue that fluctuates with the menstrual cycle), fat necrosis (firm lumps caused by injury to fatty breast tissue), and lipomas.
However, breast cancer is the most common cancer in the UK, affecting approximately 1 in 7 women. Any new breast lump, skin change, nipple change, or breast pain that persists should be assessed. The goal of breast ultrasound is either reassurance (the lump has clearly benign characteristics) or early identification of anything that needs further investigation.
What Breast Ultrasound Shows
Ultrasound is particularly effective at distinguishing solid masses from fluid-filled cysts. Simple cysts are almost always benign and often do not require any further action. Solid masses are assessed for their shape, borders, internal echogenicity, and vascularity. The BI-RADS (Breast Imaging Reporting and Data System) classification grades findings from 1 (negative) to 5 (highly suspicious), guiding whether monitoring, biopsy, or specialist referral is needed.
Breast ultrasound is especially valuable for women with dense breast tissue, which is common in younger women and makes mammograms harder to interpret. For women under 40, ultrasound is typically the first-line investigation for breast lumps. For women over 40, ultrasound often complements mammography.

Why Testicular Lumps Need Assessment
Testicular cancer is the most common cancer in men aged 15-45, with approximately 2,300 new cases diagnosed in the UK each year. The excellent news is that testicular cancer has one of the highest cure rates of all cancers — above 95% when caught early.
Most testicular lumps are not cancer. Common benign causes include epididymal cysts (fluid-filled cysts in the epididymis, the tube behind the testicle), hydroceles (fluid collections surrounding the testicle), varicoceles (dilated veins within the scrotum, often described as feeling like "a bag of worms"), spermatoceles, and inguinal hernias extending into the scrotum.
However, because testicular cancer can present as a painless, hard lump on the testicle itself, any new testicular lump should be investigated with ultrasound. The scan is quick, painless, and provides definitive information about the nature of the lump.
What Testicular Ultrasound Shows
Ultrasound clearly distinguishes between intratesticular masses (within the testicle, which require urgent assessment) and extratesticular masses (outside the testicle, which are almost always benign). It identifies cysts versus solid masses, assesses blood flow using Doppler imaging, and characterises the exact size and location of any abnormality.
For men experiencing testicular pain without a palpable lump, ultrasound can identify torsion (a medical emergency requiring immediate surgery), epididymo-orchitis (infection), and varicoceles.
What Happens After Your Testicular Scan
If the scan shows a clearly benign finding (epididymal cyst, hydrocele), you receive reassurance and advice.
