What screening can and cannot do, honestly
Screening works when it finds something common enough to be worth looking for, early enough to change the outcome, with a test accurate enough not to cause more harm than it prevents. Blood pressure, cholesterol, HbA1c and kidney function meet that bar comfortably, because the conditions they find are silent for years, enormously common, and entirely modifiable once known, and the treatment of them is the best-evidenced medicine there is. The national programmes meet it too, cervical, breast and bowel screening exist because trials showed they save lives, and any honest private clinic tells you to use them rather than replacing them. Where the bar is not met is the indiscriminate imaging of asymptomatic people, which finds a great many things that were never going to hurt anyone, and a small number that were, at the cost of investigating everyone in between. An abdominal ultrasound in a man over 65 who has never had an aortic check, a thyroid or renal scan where the bloods point at one, a testicular or breast ultrasound where the examination raises a question, a liver assessment where the enzymes and the metabolic picture warrant it, each of these is a targeted question with an answer worth having. What a screening programme should actually contain
Four things, in this order, and the order matters more than the brochure.
A doctor's assessment first. History, family history, examination, blood pressure, the questions that decide what should be tested. A programme designed before anyone has spoken to you is a product, not an assessment.
Comprehensive bloods as the backbone, because blood carries the highest yield per pound in all of screening. Cardiovascular markers, glucose and HbA1c, kidney and liver function, thyroid, full blood count, inflammation, iron studies, vitamin D, and the sex and age-specific additions, PSA where the informed choice conversation has been had, hormonal markers where relevant.
