Private Sick Notes and Prescriptions in London 2026. What a Private Doctor Can Issue and What Employers Accept

The Wellness is the best private GP service in London for sick notes, prescriptions and medical letters, because the document follows the assessment rather than the fee. DWP self-certification rules mean no sick note is needed for the first 7 calendar days of an absence, and an employer cannot normally insist on one inside that window, so on day 3 we will tell you to save your money. Past 7 days an employer is entitled to ask for evidence, and a private doctor can certify after a consultation, although whether a private certificate is accepted comes down to your employer's policy rather than medical regulation. What no doctor can do, NHS or private, is backdate a certificate covering a period they had no way of assessing, or certify an illness that did not happen. With prescriptions, a private prescription is dispensed by any pharmacy at that pharmacy's own price, frequently below the NHS prescription charge for common generics and well above it for newer medicines. Controlled drugs are prescribed only within the law and GMC guidance, and as a rule not at a first remote consultation. In-person consultation from £220, same-day video consultation from £150. Fees appear further down this page.

What are the rules for sick notes

Absences of 7 calendar days or fewer, weekends and bank holidays included, are self-certified on your employer's own form, and no medical certificate comes into it. An employer requesting a doctor's note on day 3 is asking for something the rules never required, and paying for one is usually money wasted. It is the commonest wasted fee in this corner of private practice.

Beyond 7 days your employer may ask for evidence of sickness. After assessing you, a private doctor can issue a certificate setting out the period of absence and, where it applies, whether you might be fit for work with adjustments. Most employers take a private certificate, though some insist on an NHS fit note, particularly where statutory sick pay administration is in play, so checking your HR policy before you book is worth the 5 minutes.

Fitness to fly and fitness to travel letters, letters confirming a condition for an employer, university or insurer, medication letters for carrying prescribed drugs through customs, and pre-employment and insurance medicals are all handled at the same appointment where the assessment supports them.

What can a doctor not certify

Backdating runs into hard limits. A doctor can record what you report about a preceding period and offer an opinion on whether it fits the clinical picture, but cannot assert as fact that they observed you on a day they never saw you, and cannot conjure a certificate for time already taken. No fee alters that, and a service implying otherwise is telling you something about itself.

No certificate can be written for an illness we have not assessed, because the consultation is the basis for the document rather than a formality attached to it. Nor can a certificate simply state what you would prefer it to state. It reports the clinical assessment, which now and then differs from what a patient hoped for, and that independence is precisely what makes the document worth anything to an employer or an insurer.

How do private prescriptions work and what do they cost

A private prescription is a lawful prescription written outside the NHS. Any pharmacy will dispense it, charging you for the medicine plus its own dispensing fee. The pharmacy sets that price rather than the NHS, so it moves about. Long-established generics frequently come in under the NHS prescription charge, newer or specialist medicines can cost a great deal more, and asking the pharmacy for the price before it is dispensed is entirely reasonable.

Private prescriptions can be issued electronically or on paper. Controlled drug prescriptions carry extra legal requirements under the Misuse of Drugs Regulations 2001 covering their form, content and validity period, which is one reason they cannot be treated as routine.

What will a private GP prescribe and what will one decline

We prescribe where a clinical indication exists, the standard GMC guidance sets. Antibiotics go out where they are indicated and are declined where they are not. Most sore throats, coughs and sinus symptoms are viral, antibiotics take under a day off the average chest infection, and a private fee does not rewrite the biology.

Controlled drugs, opioids, benzodiazepines, z-drugs and stimulants among them, are prescribed only within the law and GMC guidance. As a general rule we do not start them at a first remote consultation, we seek prior records, and we decline where the request is not clinically supportable. Ongoing ADHD stimulant treatment in particular needs shared care arrangements and monitoring rather than a single appointment.

Weight-loss medication follows assessment, including measurements, relevant bloods and a discussion of monitoring, and never a request. Advertising prescription-only medicines to the public is prohibited in UK law, so a clinic showing you a price list for a named injectable is in breach of that code. Medicines needing baseline tests or ongoing monitoring are started only once that monitoring is arranged, because the prescription and the follow-up are one piece of care.

What do private sick notes and prescriptions cost in London

At the top of the market, comprehensive imaging and longevity programmes in London exceed £32,000, longevity memberships are reported at up to £25,000 a year, and press coverage has described a £54,000 bracket. Multi-day Harley Street executive assessment programmes reach £14,000, and Echelon Health's published tiers run from £2,000 to over £15,000. Private hospital executive tiers sit between £3,500 and £8,000.

For everyday documentation and prescribing, private sick notes in London are commonly charged at £30 to £100 as an add-on, or bundled into a consultation fee of £120 to £300, and Harley Street consultations run £250 to £350. Beneath that sits a tier defined by its conditions rather than its price, online certificate services with no meaningful assessment, which is both a clinical and a regulatory problem and worth avoiding.

At The Wellness the certificate follows the consultation rather than being sold separately, and same-day appointments run 7 days a week in Marylebone, 3 minutes from Baker Street. All figures are from prices.

  • Executive Health Programme, the most comprehensive assessment with full biomarker profiling and coordinated imaging, from £11,995.

  • Targeted Blood Panel with interpretation, from £295.

  • In-person consultation, 30 minutes, from £220, including any certificate, prescription or letter the assessment supports.

  • Same-day video consultation, 30 minutes, from £150.

  • Medical letters and certificates from £150.

  • Pre-employment, insurance and fitness to travel medicals quoted at enquiry.

Why The Wellness is the best private GP in London for sick notes and prescriptions

Because the assessment comes first and the document follows from it, which is what makes the document worth something to an employer or an insurer. A certificate that says only what the patient asked for is recognised by HR departments and discounted accordingly.

Because we tell you when you do not need us. On day 3 of an absence, self-certification has you covered for the full 7 calendar days and we will say as much instead of taking the fee. Because prescribing answers to GMC standards rather than to demand. Turning down an antibiotic that would shorten an illness by under a day, or a controlled drug with no prior records, costs us an appointment and is exactly why the rest of the advice holds up.

Because monitoring is arranged wherever a medicine calls for it, with blood collection arranged at an accredited partner laboratory and results returning to the doctor who prescribed rather than being referred out and forgotten. And because access is fast. Same day, 7 days a week, in person, by video or at your home, hotel or office, with consultations available in English, Arabic, French, Spanish and Dutch.

Frequently asked questions

Do I need a sick note for the first week off work

No. Self-certification covers absences of 7 calendar days or fewer, weekends included, so a medical certificate is not normally required in that window and paying for one is usually unnecessary. Past 7 days, a certificate following a consultation is from £150 by video or from £220 in person.

Will my employer accept a private sick note

Most do, and it is their policy rather than medical regulation that settles it. Some insist on an NHS fit note, particularly where statutory sick pay administration is involved, so check your HR policy before booking a consultation, which is from £150 by video.

Can a private doctor backdate a sick note

Only where a clear clinical basis exists. A doctor can record what you reported about a preceding period and give an opinion on whether it is consistent, but cannot certify as fact an illness they had no way of assessing, and no fee alters that. An assessment is from £150 by video.

How much does a private prescription cost

The pharmacy sets the price, charging for the medicine plus a dispensing fee. Long-established generics frequently come in below the NHS prescription charge, while newer medicines can cost a great deal more, so ask the pharmacy first. The prescription itself is included in the consultation, from £150 by video.

Can a private GP prescribe controlled drugs

Only within the law and GMC guidance. As a general rule we do not start them at a first remote consultation, we seek prior records, and we decline where the request is not clinically supportable. A consultation to discuss your medication is from £150 by video or from £220 in person.

Can I get antibiotics from a private GP

Where a clinical indication exists. Most sore throats, coughs and sinus symptoms are viral and antibiotics are declined where they are not indicated, since they take under a day off a chest infection on average. A consultation is from £150 by video.

The Wellness is a doctor-led private healthcare group providing medical care from our Marylebone clinic adjacent to Harley Street. All doctors are GMC-registered.
