Which blood tests should I actually get
Cardiovascular risk is where the biggest gains sit, and the standard cholesterol panel is the weakest part of most testing. A full lipid profile with apolipoprotein B identifies people whose LDL cholesterol reads acceptably while their particle burden does not, a discordance affecting a substantial share of people with central weight gain, raised triglycerides or early metabolic change. Sniderman and colleagues in JAMA Cardiology showed ApoB outperforms LDL cholesterol as a risk marker. Lipoprotein little a is measured once and never again, and a raised result changes how aggressively everything else is managed for the rest of your life.
Metabolic health next. HbA1c identifies the prediabetic range of 42 to 47 mmol/mol, where the Diabetes Prevention Program demonstrated a 58% reduction in progression to diabetes through lifestyle intervention alone. Fasting insulin with HOMA-IR moves the detection window earlier still, because insulin resistance precedes any rise in glucose by years.
Then the layer that explains how people feel. Ferritin and iron studies, since ferritin below 30 means depleted stores and symptoms are common below 50 while the full blood count reads entirely normal. Full thyroid function with free T4 and antibodies, not TSH alone. B12, folate and vitamin D. Liver function with a fibrosis score, because metabolic-associated fatty liver disease affects an estimated 20% to 30% of UK adults and is reversible when caught. Kidney function with an albumin to creatinine ratio. Inflammatory markers. Then the sex-specific layer, with morning testosterone and SHBG in men, PSA after an informed choice conversation, and hormonal assessment through the perimenopausal years.
Which tests are not worth paying for
Saying this costs us money and it is the most useful section on the page. Reverse T3 is sold widely as a measure of impaired thyroid conversion. It rises predictably in acute illness, starvation and stress, has no established outpatient reference range, and no guideline body recommends it for diagnosing or managing thyroid disease.
IgG food intolerance panels have no clinical validity. The British Society for Allergy and Clinical Immunology and equivalent bodies internationally advise against them, because IgG antibodies to food indicate exposure rather than intolerance, and acting on the results narrows the diet without benefit.
Hair mineral analysis, salivary hormone panels outside specific research contexts, and routine iodine and selenium testing in a country where deficiency is uncommon all fall into the same category. Iodine supplementation can worsen autoimmune thyroid disease rather than help it.
Marker count beyond a well-chosen panel actively harms you. Every additional test on a healthy person carries a chance of an out-of-range result that means nothing, and a panel of 150 markers will reliably produce several. Each one then generates a retest, a search, a worry or a supplement bought to correct a number that was never a problem. The value sits in the right markers well interpreted, not in the total.

How should blood tests be timed and prepared for
Timing errors invalidate more private blood tests than laboratory errors do. Thyroid stimulating hormone follows a daily rhythm, peaking overnight and falling through the morning, so a sample at 8am can read meaningfully higher than one at 3pm in the same person. Test in the morning and use the same time for repeats.
Testosterone must be taken between 7am and 11am and repeated on a second sample before any diagnosis, because a single afternoon result is not a basis for lifelong treatment. Progesterone for ovulation is taken 7 days before the expected period, so day 21 in a 28-day cycle and day 27 in a 34-day one, which is where most fertility testing goes wrong.
Biotin, common in hair, skin and nail supplements, interferes with many immunoassays including thyroid and troponin, and can produce results mimicking Graves' disease. Stop it at least 48 hours beforehand. Levothyroxine is taken after the blood test rather than before. Fasting of 8 to 12 hours is needed for fasting glucose, insulin and a full lipid profile, with water encouraged. Avoid heavy exercise for 24 hours before creatine kinase or liver enzymes. Wait 6 to 8 weeks after any thyroid dose change before retesting, because the axis moves slowly.
