The thyroid is a small, butterfly-shaped gland at the front of your neck that produces hormones controlling your metabolism, energy levels, body temperature, heart rate, weight, and mood. When the thyroid malfunctions, the effects are felt throughout the body.

Thyroid disorders are remarkably common in the UK. Approximately 1 in 20 people have some form of thyroid condition, with women affected roughly five to eight times more often than men (British Thyroid Foundation, 2025). Many cases go undiagnosed for years because the symptoms are vague, develop gradually, and overlap with other common conditions such as depression, chronic fatigue, stress, and perimenopause.

An underactive thyroid (hypothyroidism) causes fatigue, weight gain, cold sensitivity, dry skin, hair thinning, constipation, low mood, and brain fog. An overactive thyroid (hyperthyroidism) causes weight loss, anxiety, tremor, palpitations, heat intolerance, sweating, and irritability. Thyroid nodules affect up to 50% of people by age 60 (most are benign and cause no symptoms), but approximately 5% of nodules harbour thyroid cancer, which is why investigation is important.

When You Should Get a Thyroid Check

There are several clinical scenarios where a thyroid assessment, including ultrasound, is clearly warranted.

You have noticed a lump or swelling in your neck. Any new neck lump should be investigated. A thyroid ultrasound can rapidly determine whether the lump is arising from the thyroid (nodule, cyst, or goitre) or from another structure (lymph node, salivary gland, or other). Most thyroid lumps are benign, but prompt assessment provides reassurance and identifies the rare cases that need further investigation.

Your thyroid blood tests are abnormal. If a blood test has shown elevated or suppressed TSH, abnormal free T3 or free T4, or positive thyroid antibodies (anti-TPO, anti-thyroglobulin), an ultrasound of the thyroid provides structural information that complements the functional data from blood tests.

You have symptoms of thyroid dysfunction that are unexplained. Persistent fatigue, unexplained weight changes, hair loss, mood disturbances, palpitations, or temperature sensitivity that do not have a clear cause warrant thyroid investigation. Even if blood tests appear normal, an ultrasound may reveal structural changes (such as thyroiditis or nodules) that explain your symptoms.

You have a family history of thyroid disease or thyroid cancer. First-degree relatives of people with thyroid cancer or autoimmune thyroid disease are at higher risk and may benefit from baseline screening.

You have difficulty swallowing or a sensation of throat pressure. A large goitre or thyroid nodule can compress the oesophagus or trachea, causing these symptoms. Ultrasound determines the cause and guides further management.

A thyroid abnormality was found incidentally. If a scan performed for another reason (such as a carotid Doppler or neck CT) identified a thyroid incidentaloma, a dedicated thyroid ultrasound provides more detailed assessment.

Thyroid nodules are extremely common. Studies using high-resolution ultrasound detect nodules in up to 50% of the adult population by age 60 (British Thyroid Foundation, 2025). The vast majority, approximately 95%, are benign. They require monitoring but not treatment.

Thyroid cancer accounts for approximately 3,900 new cases per year in the UK (Cancer Research UK, 2025). The most common form (papillary thyroid cancer) has an excellent prognosis, with a 10-year survival rate exceeding 95% when detected and treated early.

Ultrasound cannot definitively diagnose thyroid cancer, but it can identify features that indicate a higher risk and guide the decision about whether fine needle aspiration biopsy is needed. The British Thyroid Association's U-classification system (U1 to U5) is used to categorise nodules based on their ultrasound appearance, with U1 being normal and U5 being highly suspicious.
