Well Woman Health Checks in London 2026. What to Test, What the NHS Now Covers and What Nobody Screens For

Women's health screening in Britain changed substantially in the last eighteen months and most women have not been updated. From July 2025, women aged 25 to 49 in England who test negative for HPV are invited for cervical screening every five years rather than three, bringing England in line with Wales and Scotland, and the change is safe because the HPV test that now leads the programme is considerably better than the cytology it replaced. From January 2026, NHS England began sending home HPV self-sampling kits to women more than six months overdue for screening, a response to the five million women not up to date, and modelling suggests it could save several thousand lives a year. Both changes are good, both are free, and the first rule of sensible private screening is to use every national programme you are invited to, cervical from 25, breast from 50, bowel from 50. What those programmes do not cover is most of what actually troubles women through their thirties, forties and fifties. Iron deficiency, extraordinarily common after decades of menstruation and the single most missed cause of exhaustion. Thyroid disease, which affects women far more than men and mimics everything. Perimenopause, which begins years before periods stop and is routinely mistaken for stress. Bone health, which starts declining around menopause with no symptoms until a fracture. And the ovarian question, which deserves the straight answer this page gives it rather than the CA125 blood test too many clinics sell for it. What the national programmes cover, and how to use them properly

‍ ‍

Cervical screening runs from 25 to 64 and now tests for high-risk HPV first, with cell examination following only where the virus is found, which is why a negative HPV result is so reassuring that five-yearly recall is safe for women aged 25 to 49. Women who test positive stay on closer follow-up. If you are overdue, the home self-sampling kit now being posted to under-screened women removes most of the reasons people give for not attending, and it is worth accepting. The HPV vaccine, given in schools since 2008 for girls and 2019 for boys, prevents up to 90 percent of cervical cancers and has contributed to a substantial fall in incidence, and catch-up vaccination is available into adulthood for those who missed it. Breast screening invites women from 50 to 71 every three years, and the crucial caveat is that a mammogram between invitations does not exist in the programme, so a new lump, skin or nipple change, or one-sided pain needs assessment now rather than at the next round. Bowel screening now begins at 50 across England with a home FIT kit every two years, and it is the easiest test in medicine to complete and among the most effective. Use all three. Private screening should fill the gaps around them, not duplicate them, and any clinic selling you a private smear when your NHS invitation is due is selling you your own money back.

The ovarian cancer question, answered straight

‍ ‍

This is where women are most often misled, so here is the evidence. CA125 is a blood marker that rises in ovarian cancer and also in endometriosis, fibroids, pelvic infection, liver disease, pregnancy and simply having a period, which is why it performs poorly as a screening test in women without symptoms. The largest trial ever conducted on this question, following more than 200,000 women in the UK for two decades using CA125 with ultrasound, did not show a reduction in ovarian cancer deaths, which is why there is no national ovarian screening programme and why the responsible position is to say so. What does help is knowing the symptoms, because ovarian cancer is not silent, it is vague. Persistent bloating, feeling full quickly, pelvic or abdominal pain, and needing to pass urine more urgently or frequently, occurring on most days for three weeks or more, warrant assessment, and in that setting CA125 and pelvic ultrasound are useful because they are being used to investigate rather than to screen. Family history changes the picture entirely, and a pattern of breast, ovarian, pancreatic or prostate cancer across relatives is a reason for genetic assessment and, where a BRCA variant is confirmed, a formal surveillance pathway, which is a different conversation from a wellness panel.

What a woman should be tested for, by decade

‍ ‍

In the thirties, the priorities are iron and thyroid, plus vitamin D, because deficiency is common and its symptoms, exhaustion, hair shedding, low mood, poor concentration, are routinely attributed to modern life. Fertility questions belong here too, and hormonal assessment where cycles are irregular or pregnancy is being planned. Blood pressure begins to matter. In the forties, add the cardiovascular and metabolic layer, a full lipid profile with apolipoprotein B, lipoprotein little a measured once in your lifetime, HbA1c, and liver and kidney function, because women's cardiovascular risk is persistently underestimated and rises sharply after menopause. Perimenopause typically begins in the mid to late forties and is diagnosed clinically rather than by blood test in women over 45, which is worth knowing before anyone sells you a hormone panel to prove it. In the fifties and beyond, bone health enters, since density falls fastest in the years around menopause and a DEXA scan is the right test where risk factors exist, and the cardiovascular numbers matter more than ever. Throughout, imaging is aimed rather than routine, pelvic ultrasound where symptoms, cycles or examination raise a question, breast ultrasound where a lump needs characterising, thyroid or abdominal scanning where the bloods point there, rather than annual whole-body scanning the evidence does not support.

Do I need a hormone blood test to diagnose perimenopause

Over 45 with typical symptoms, no. Guidance is explicit that the diagnosis is clinical, because fluctuating hormone levels in this window mislead. Testing matters under 45, where premature ovarian insufficiency must be confirmed, and to check thyroid and iron, which mimic the same picture.

When should I get a lump or change checked

‍ ‍

Now, not at your next screening round. Breast screening runs three-yearly from 50 and does not cover the time between invitations, so a new lump, skin or nipple change, or one-sided pain deserves assessment promptly, with ultrasound available here in the same visit.
