Imaging is arranged at the London centre best equipped for each study, performed by a sonographer or radiographer and reported by a consultant radiologist. Blood collection is arranged separately at an accredited partner laboratory and compressed into the same day where diaries allow. A review appointment with the doctor who assessed you reads everything together, and anything needing a gynaecologist, breast consultant or cardiologist goes to a named consultant with the appointment booked.

Which tests change something at 40, 50 and 60

At 40 the highest-yield tests are in the blood. Ferritin below 30 micrograms per litre is iron deficiency under the British Society of Gastroenterology guideline whatever the haemoglobin says, and in the PREFER trial iron infusion reduced fatigue in 65% of iron-deficient women against 53% on placebo. A mildly raised TSH is repeated at 3 months before anyone is treated, as NG145 requires, because many such results normalise on their own. Lipoprotein little a is measured because 1 in 5 women carry an inherited level a standard panel never sees.

At 50 the CT coronary calcium score earns its place, because women's heart disease is under-investigated and the MESA data held across sex. A woman of 52 with a normal LDL, a raised lipoprotein little a and a calcium score of 90 is a treatment decision a lipid panel alone would have missed. The abdominal ultrasound and FIB-4 score find the fatty liver that affects 1 in 4 adults according to Younossi and rises after the menopause. NHS breast and bowel screening both start at 50, and we tell every woman to attend both.

At 60 DEXA joins the list, because 1 in 2 women over 50 will break a bone through osteoporosis according to the Royal Osteoporosis Society and the first fracture is often the first symptom. It is offered earlier with an early menopause, a maternal hip fracture, long steroid use or a low body weight, and the FRAX score decides whether treatment follows. Thyroid ultrasound is added only for a lump, a symptom or an abnormal result.

Which hormone tests should you not pay for

A hormone panel to diagnose perimenopause in a woman over 45 is the clearest example. NICE guideline NG23 says the diagnosis is made on symptoms alone in that age group, because FSH and oestradiol swing from week to week and a single result proves nothing either way. The result changes neither the diagnosis nor the treatment. Under 40 with absent periods FSH is part of the work-up, which is the distinction a doctor makes before the sample is taken.

An AMH level sold as a fertility forecast is the second. AMH reflects egg number, not egg quality, and the study by Steiner and colleagues in JAMA in 2017 found that women with low AMH were no less likely to conceive naturally over 12 months than women with normal levels. It has a place in planning IVF or egg freezing, where it predicts response to stimulation, and no place as a stand-alone number for a 34-year-old deciding whether to worry. Testosterone in a woman without low sexual desire on optimised HRT is the third, since NICE reserves it for that indication.

Which scans should you not buy

A pelvic ultrasound without symptoms is not a screening test. Fibroids are found in most women by 50 and the majority never need treatment, and ovarian cysts come and go with the cycle, so a routine scan finds things that lead to further scans rather than better health. Pelvic ultrasound earns its place for heavy or irregular bleeding, pain, persistent bloating or a fertility question, and in each it is arranged within the week and reported by a consultant.

Whole-body MRI for a healthy 40-year-old with no family history is the second, since the systematic review of 12 studies and 5,373 people found no demonstrated improvement in outcomes. A breast MRI or ultrasound bought privately without a breast consultant to read it in context is the third. Where breast density, a family history or a symptom justify imaging, the referral goes to a named breast consultant with the appointment booked.
