But medication does not change the basic physiology of losing weight. Why appetite is biological as well as behavioural

Hunger and fullness are regulated through a complex interaction between the gastrointestinal system, hormones and the brain. These signals influence when we become hungry, how strongly we experience appetite and when we feel satisfied after eating. This is one reason sustained weight loss is more complicated than simply deciding to eat less.

Modern weight management medications target some of these pathways and can reduce appetite or increase feelings of fullness. For the right patient, this can make maintaining an appropriate energy intake considerably more manageable than relying on conscious restriction alone.

That does not mean medication is appropriate for everyone, nor does requesting treatment automatically make someone eligible. Why losing weight is not the same as losing fat

The number on a set of scales cannot distinguish between body fat, muscle, fluid and other components of body mass. During significant weight loss, some loss of lean tissue can occur alongside the reduction in fat mass. Clinical studies involving modern weight management treatments have demonstrated this clearly, which makes body composition an important part of understanding the quality of weight loss rather than focusing exclusively on total kilograms lost.

Preserving muscle matters for reasons that go beyond appearance. Skeletal muscle contributes to physical function, strength and metabolic health, and losing unnecessary amounts of lean tissue can make maintaining the eventual result more difficult. Resistance training and adequate dietary protein therefore become particularly important during a substantial period of weight reduction.

What happens when weight loss medication is stopped

One of the most important findings from longer-term studies of modern weight management medication is what happens after treatment is withdrawn. Appetite regulation does not necessarily remain permanently altered once the medication is stopped, and significant weight regain has been observed in people after treatment discontinuation.

This should not be interpreted simply as a failure of the medication. Obesity and excess weight can behave as chronic, relapsing conditions, and the biological pressures that contributed to weight gain may become more apparent again when pharmacological treatment is removed.

The practical implication is that stopping treatment deserves as much planning as starting it. A maintenance phase can involve continued nutritional support, regular monitoring, resistance training, adjustment of energy intake and, where clinically appropriate, decisions about medication made with the treating physician.

No responsible clinic can guarantee that someone will never regain weight. What can be done is to avoid treating the end of medication as the end of the programme and to build a strategy for maintaining the result before treatment is withdrawn.

Who medical weight management may be appropriate for

Medical weight management is generally intended for adults whose weight, BMI and wider clinical circumstances make treatment appropriate. Eligibility is not determined by appearance alone or by a patient's desired target weight, and medication should not simply be regarded as a shortcut for modest cosmetic weight loss.

The initial assessment is also an opportunity to consider whether another health issue could be contributing to weight gain or making weight management more difficult. Depending on the individual, relevant assessment may include metabolic health, thyroid function, glucose regulation, cardiovascular risk, medication history and other factors.

Side effects, contraindications and treatment suitability differ between individuals. Outcomes also vary substantially, which is why a clinician cannot responsibly promise that a particular patient will lose a fixed number of kilograms or reach a specific target by a particular date before assessing them.

The useful promise is therefore a clinical process rather than a guaranteed result: appropriate assessment, medical monitoring, adjustment where required and a plan developed around the individual rather than a generic prescribing protocol.

What medical weight management costs at The Wellness

Private medical weight management in London ranges from relatively inexpensive online prescribing models to comprehensive consultant-led programmes involving blood testing, nutrition, body composition assessment and ongoing clinical supervision. At The Wellness, the programmes are structured around medical oversight, nutrition, strength support and longer-term maintenance rather than medication being provided in isolation.

The Foundation costs £595 a month. It includes physician-led care, medication where clinically appropriate, a biomarker panel during the first month and repeated every six months, monthly consultations with the lead physician, a nutritionist-designed meal plan, a coach-designed strength programme and private WhatsApp access.

The Composition costs £945 a month and builds on The Foundation with monthly video consultations with the physician, monthly consultations with a personal nutritionist and body composition scanning during the first month and then every six months.

The Optimisation costs £2,150 a month and is the most comprehensive programme. It builds on The Composition with two sessions each month with a personal strength coach, two nutritionist consultations each month, quarterly body composition scanning, quarterly comprehensive biomarker testing, continuous glucose monitoring where clinically indicated and priority WhatsApp access.

Why body composition and maintenance matter

A successful weight management programme should look beyond the scale. Someone can lose a substantial amount of total body weight while also losing more muscle than intended, while another person may experience a smaller change on the scale but a favourable change in body composition. Measuring the process helps clinicians distinguish between those outcomes.

This is why body composition assessment becomes particularly useful in a longer programme. It provides an additional way of monitoring whether changes are occurring predominantly in fat mass, lean tissue or both and allows nutrition and strength strategies to be adjusted accordingly.

The same longer-term thinking applies to maintenance. The period after substantial weight loss should not be regarded as an afterthought. Habits, appetite, training, nutrition and medication strategy may all need to evolve as someone approaches their target and moves from active weight loss to maintaining a relatively stable weight.
