Do I need a GP referral to see a private specialist?

In most cases, yes. The majority of private consultants in the UK require a GP referral letter before they will see a patient. This is not a bureaucratic formality. The referral letter provides the specialist with your clinical history, current symptoms, relevant examination findings, investigation results, and the specific clinical question the GP is asking the specialist to address. This allows the specialist to prepare for your appointment and use the consultation time efficiently.

Some specialists will see patients without a GP referral (known as self-referral), but this is the exception rather than the norm. Even where self-referral is possible, many private medical insurance policies require a GP referral for the consultation to be covered.

Any GMC-registered GP can write a referral letter to any private specialist. You do not need to go through your NHS GP. A referral from a private GP carries the same clinical weight as one from an NHS GP.

Why are NHS specialist waiting times so long in 2026?

The NHS referral-to-treatment (RTT) waiting list reached approximately 7.5 million pathways by early 2026 according to NHS England data. The median wait for consultant-led treatment is over 14 weeks, with some specialties significantly longer. According to the British Medical Association, over 300,000 patients have been waiting more than a year for treatment, and tens of thousands have waited over 18 months.

Diagnostic capacity (MRI, CT, endoscopy) is constrained. Elective surgery cancellations due to bed pressures and industrial action have compounded the problem.

For patients with progressive conditions, significant pain, diagnostic uncertainty, or conditions where delay risks worse outcomes, waiting months is not clinically acceptable. Orthopaedics. Hip pain, knee pain, shoulder injuries, back pain, sports injuries, joint replacement assessment. NHS orthopaedic waits are among the longest in the system. Private orthopaedic consultants can typically see you within one to two weeks.

Can I choose which specialist I see?

Yes. You can request a referral to a specific consultant by name, or ask the GP to recommend a suitable specialist based on your clinical needs. Many patients research consultants themselves through platforms such as Top Doctors, Doctify, or hospital websites and arrive at the consultation with a preferred consultant in mind. The GP will write the referral letter addressed to that consultant.

How much does a private referral cost?

Video consultation including referral letter from £250. This is the standard route. The GP conducts a clinical assessment, discusses your symptoms, and writes a detailed referral letter addressed to the specialist of your choice. The letter is emailed as a PDF on the same day.

Telephone consultation including referral letter from £150. May be suitable for straightforward referrals where the clinical picture is clear and the patient has existing documentation.

In-person consultation including referral letter from £295. Recommended where the GP needs to conduct a physical examination before writing the referral.

Blood tests or investigations before referral from £195. Some specialists prefer to receive blood test results or imaging with the referral letter. The GP can arrange these at the same time, which accelerates the specialist appointment.

The referral letter is included within the consultation fee. There is no separate charge for the letter.

What does a good referral letter contain?

The quality of the referral letter directly affects the quality of the specialist appointment. A detailed, well-structured referral allows the specialist to prepare in advance, focus the consultation on the key clinical questions, and avoid repeating investigations unnecessarily.

Every referral letter from The Online GP includes a clear summary of the presenting complaint and symptom timeline, relevant medical history and current medications, examination findings from the GP assessment, results of any blood tests or investigations already performed, the specific clinical question the GP is asking the specialist to address, and the patient's contact details and preferred method of communication.

For insurance purposes, the letter confirms that the referral is for an open referral (allowing the patient to choose their provider) or to a named consultant, and includes the GP's GMC registration number and clinic details.

Will my private medical insurance cover the referral?

Most private medical insurance policies (Bupa, AXA Health, Aviva, Vitality, WPA, Cigna, and others) require a GP referral letter for specialist consultations to be covered. A referral from a private GP is accepted by all major UK insurers on the same basis as a referral from an NHS GP.

Some insurers require pre-authorisation before you see the specialist. The GP referral letter provides the clinical information the insurer needs to authorise the claim. The process

2. GP consultation. A GMC-registered doctor conducts a clinical assessment (video, telephone, or in-person). The doctor discusses your symptoms, takes a history, and determines the most appropriate specialty and consultant.

3. Referral letter issued. Emailed as a PDF on the same day. The letter is addressed to the named consultant or to the relevant department at a named hospital.

4. Book with the specialist. You contact the specialist's secretary or the hospital directly to book. Most private consultants can see you within one to two weeks. We can provide contact details for recommended specialists.

Important information

A referral is not guaranteed. If the GP's assessment does not support a specialist referral (for example, if the condition can be fully managed at GP level, or if the GP identifies a different clinical pathway), a referral will not be made. The GP will explain their reasoning and recommend the appropriate next steps.
